摘 要:目的了解老年女性泌尿系感染病原菌的分布及耐药情况,指导临床合理用药。方法对医院2010~2012年泌尿系感染的老年女性患者中段尿进行细菌培养及药敏分析。结果医院老年女性泌尿系感染的细菌以革兰氏阴性菌为主,占59.2%,常见致病菌有大肠埃希菌、粪肠球菌、肺炎克雷伯菌和金黄色葡萄球菌。真菌感染比例较高,为23.1%.大肠埃希菌和肺炎克雷伯菌对亚胺培南敏感,对头孢哌酮/舒巴坦和阿米卡星的耐药率较低,为4.8%~23.8%,而对其它抗菌药物耐药率较高;革兰阳性球菌对万古霉素敏感,对阿米卡星和呋喃妥因的耐药率低,为2.6%~17.9%。结论泌尿系感染病原菌的培养和耐药性监测,对临床合理用药具有重要意义。Objective To understand the distribution and antimicrobial resistance of pathogenic bacteria in urinary tract infection in old female to guide the clinician to reasonably use of antibiotics. Methods The bacterial culture and susceptibility testing were per- formed for the isolates from mid-stream urine specimen from 2010 to 2012. Results Urinary tract infection of old female is mainly caused by Gram-Negative Bacillus, which takes 59. 2 % , other pathogens are also observed frequently such as Escherichia coli, Entero- coccus faecalis, K. pneumoniae, Staphylococcus aureus. The isolating rate of fungus is 23. 1%. Escherichia coli and K. pneumoniae are sensitive to imipenem. The resistance rates of cefoperazonc/sulbaetam and amikacin against E. coli and K. pneumoniae are lower, var- ying from 4. 8 % ~ 23.8%. The resistance to the other antibiotics are the higher. Gram positive coccus is sensitive to vancomycin. Those of amikacin and nitrofurantoin against gram positive coccus are the lower,varying from 2. 6% ~ 17. 9%. Conclusion It is important that the clinician reasonably use the antibiotics according to the bacterial culture and susceptibility testing.
