357例冠心病心衰患者中医证候与心功能分级的相关性研究

摘　　要：目的探讨冠心病心衰患者中医证候与心功能分级之间的相关性。方法收集冠心病心衰患者357例,以中医证候为自变量,以心功能分级为因变量,进行二值Logistic回归分析。结果与心功能Ⅲ级、Ⅳ级相比,心功能Ⅱ级患者出现水停证、痰浊证、阴虚证、阳虚证的可能性较小;心功能Ⅲ级患者出现水停证、阴虚证、痰浊证的可能性为心功能Ⅱ级、Ⅳ级患者的2.343倍、2.226倍、4.217倍;心功能Ⅳ级患者出现血瘀证、水停证、痰浊证、阳虚证的可能性为心功能Ⅱ级、Ⅲ级患者的8.012倍、2.800倍、2.295倍、5.073倍;冠心病心衰Ⅲ级、Ⅳ级发生血瘀证、水停证、痰浊证、阳虚证的可能性较大。结论冠心病心衰患者中医证候与心功能分级具有一定的相关性。Objective To investigate the correlation between traditional Chinese medicine（TCM） syndromes and cardiac function in patients with coronary heart disease and heart failure. Methods Three hundreds and fifty - seven patients （pts） with coronary heart disease and heart failure were studied. TCM syndrome as independent variables and heart function classified as dependent variables were studied by binary logistic regression analysis. Results Compared with pts with cardiac function Ⅲ and Ⅳ grade,water retention syndrome,phlegm syndrome, yin deficiencysyndrome,yang deficiency syndrome were less in pts with cardiac function Ⅱ . The oppor tunity of water stopping syndrome,yin deficiency syndrome,and phlegm syndrome in pts with cardiac function grade Ⅲ were 2. 343, 2. 226,4. 217 times respectively,compared with that in pts with cardiac function grade Ⅱ and Ⅳ. The possibility of blood stasis syn drome,water retention syndrome, phlegm syndrome,and yang deficiency syndrome in patients with NYHA Ⅳ were 8. 012,2. 800,2. 295, and 5. 073 times respectively,compared with that in patients with heart function class Ⅲ and Ⅲ. Morbidity of stasis syndrome, water retention yndrome, phlegm syndrome,and yang deficiency syndrome in patients with heart failure grade Ⅲ and Ⅳ were popular. Con- clusion TCM syndromes were related with cardiac function classification in coronary heart disease patients with heart failure.
