持续皮下输注胰岛素治疗2型糖尿病的临床研究

摘　　要：目的探讨胰岛素泵持续皮下输注胰岛素治疗2型糖尿病的有效性及安全性。方法将100例经口服降糖药物治疗仍血糖控制不佳的2型糖尿病患者随机分为两组,每组50例。观察组采用胰岛素泵持续皮下输注胰岛素治疗,对照组采用三餐前皮下注射胰岛素治疗,对两组的疗效及不良反应进行比较分析。结果观察组血糖达标时间、胰岛素用量及血糖稳定期的胰岛素使用剂量均明显低于对照组,差异有统计学意义(P<0.05)。观察组低血糖、黎明现象的发生率均低于对照组,但两组差异无统计学意义(P>0.05)。两组治疗后的空腹血糖、餐后2小时血糖及血红蛋白水均比治疗前明显下降(P<0.05),但观察组下降更显著(P<0.01),且观察组与对照组的差异亦分别有统计学意义(P<0.05)。结论胰岛素泵持续皮下输注胰岛素治疗2型糖尿病安全有效,低血糖反应及黎明现象等副反应的发生率低。Objective To study the efficacy and safety about continons subcutaneous insulin injection in the treatment of type 2 diabetes. Methods 100 cases of type 2 diabetic patients received oral hypoglycemic drug treatment and poor glucose control were randomly divided into the observation group and the control group. 50 patients in the observation group were treated with continuous subcutaneous insulin infusion therapy, the other 50 patients in the control group were treated with subcutaneous insulin injections therapy before meals. The efficacy and adverse reactions of two groups were compared and analyzed. Results In the observation group, the blood glucose time, insulin dosage and insulin dosage at blood sugar stable stage of were statistically lower than those in the control group （P 〈0.05）. The hypoglycemia reactive and dawn phenomenon of observation were lower than those of the control group, but with no significant difference between the two groups （P 〉0.05）. Atter treatment, the fasting blood glucose, postprandial 2 hour blood glucose and hemoglobin levels of the two groups significantly decreased （P 〈0.05）, but the observation group decreased even more significantly （P 〈0.01）, and those indicators were also significantly different between the two groups （P 〈0.05）. Conclusions Continuous subcutaneous insulin infusion is safe and effective in the treatment of type 2 diabetes, with low incidence of the deputy reaction such as hypoglycemia and dawn phenomenon.
