摘 要:目的本研究借鉴微观辨证理念,研究围塌陷期股骨头坏死股骨头内的生物力学状态,建立微观辨证论治体系。方法主要从两方面进行研究,一是股骨头坏死"微观辨证"的方法学研究;二是将临床和影像表现结合构建微观辨证论治体系。结果建立了包括证型分类和相应治疗方案的围塌陷期微观辨证论治体系。结论本研究优化了保髋路径,有助于提高保髋成功率,具有较高的临床应用价值;同时也为进一步有限元分析提供依据和线索。Objective To establish the microcosmic syndrome differentiation and treatment system (MSDTS) for osteonecrosis of the femoral head (ONFH) by applying the concept of microcosmic differentiation and analysing the biomechanics of the necrotic femoral head during peri-collapse stage (PCS) with CT and MRI. Methods The research was conducted in the following two parts: the first was the methodological research of microcosmic differentiation of ONFH; the second was the construction of MSDTS based on clinical and imaging features. Results The MSDTS for ONFH in PCS was established, which has a unique classification of microcosmic syndromes and appropriate treatment proposal based on clinical and imaging findings. Conclusions The present study has a high clinical application value, which can optimize the clinical path and facilitates, enhance the success rate of hip joint preserving for ONFH, and provide basis and clues for further finite element analysis on ONFH.
