静息心电图正常的冠心病患者256层螺旋CT检查特点和临床特征

摘　　要：目的探讨静息ECG正常的冠心病患者冠状动脉病变特点及临床特征。方法选择有冠心病典型症状及危险因素而ECG正常的患者,男58例,女20例,平均年龄(51.35±8.76)岁,对所有患者进行256层螺旋CT检查检查。结果 78例患者中,256MSCT阳性者47例(59.5%),其中男37例,女10例;单支病变26例(55.3%),双支病变14例(29.8%),3支病变7例(14.9%),其中前降支病变发生率最高(36例次,占76.6%),其次为右冠病变(22例次,占46.8%)。临床特征中256MSCT阳性者胸痛、气急症状发生率明显高于256MSCT阴性者;在主要冠心病主要危险因素中,高脂血症和性别在256MSCT阳性与256MSCT阴性患者中差异有统计学意义(P<0.05)。结论对于具有明显冠心病临床症状、同时存在二项以上危险因素的患者,尤其是男性,即使静息ECG正常,也要引起临床高度重视,必要时进行256层螺旋CT检查。Objective To investigate the 256 multi-slice computed tomography（256MSCT） features and clinical characters of coronary heart disease（CHD） patients with normal rest electrocardiogram（ECG）.Methods Seventy-eight patients [58 males and 20 females,mean age（51.35±8.76） years] with normal rest ECG were selected.All patients had typical symptoms and risk factors of CHD,and all of them were examined with the 256MSCT.Results In all of the patients,47 cases（37 males and 10 females） had positive performance of 256MSCT,26 cases（55.3%） with single vessel lesion,14 cases（29.8%） with double vessel lesions,7 patients（14.9%） with 3 vessel lesions.The left anterior descending had highest incidence（36 cases,76.6%） of artery lesions,followed by the right coronary lesions（22 cases,46.8%）.The clinical features,such as chest pain,shortness of breath in patients with positive performance in 256MSCT were significantly higher than those in patients with negative performance in 256MSCT;There was significant difference in hyperlipidemia and gender between the patients with positive performance in 256MSCT and those with negative performance in 256MSCT（P＆lt;0.05）.Conclusion For the patients with obvious clinical symptoms and two or more risk factors of coronary heart disease,especially men,they should be paid more attention to even if they have normal rest ECG.If necessary,the 256MSCT examination could be performed.
