摘  要:目的:分析该院627例地高辛血药浓度监测结果,为临床合理用药提供参考。方法:用荧光偏振免疫法测定地高辛血药浓度(SDC),结合临床资料进行统计和分析。结果:在监测的627例使用地高辛的病例中,血药浓度<0.5μg/L的有125例(19.9%),有2例(5.4%)出现中毒症状;在有效浓度范围内0.5~2.0μg/L的有445例(70.9%),出现中毒症状有5例(13.5%);血药浓度>2.0μg/L的患者有57例(9.0%),出现中毒症状有30例(81.0%)。性别对地高辛血浓度无显著影响。随着年龄增大,地高辛血浓度呈增高的趋势。中毒的临床表现以消化道反应(74.3%)为主,中毒原因有合并用药、不同疾病等。结论:SDC个体差异较大,影响因素较多。不同病因、病理生理因素以及合用药物均能影响血清地高辛浓度。设计个体给药方案是确保临床效果和用药安全的重要措施。药师在地高辛的合理用药中应发挥更多的作用。Objective:To analyze the resuh of the serum digoxin concentration in order to offer reference about clinical safety and rational us.Methods:The SDC was determined by fluorescence polarization immunization,and the results were subjected to statistical analysis by taking into consideration of clinical data.Results:The total 627 cases who treated with digoxin,the blood drug level in 125 cases ( 19.9% )were under 0.5μg/L, with 2case (5.4%)having symptoms of poisoning, 445 cases (70.9%)between 0.5 and 2.0μg/L, with 5cases (13.5%) had symptoms of poisoning, 57eases (9.0%)higher than 2.0μg/L, with 30cases (81.0%)having symptoms of poisoning. Gender had no effect on the blood drug levels,while with the age increase,the serum concentrations were raised accordingly.The clinical manifestations of poisoning are mainly gastrointestinal reactions (78.3%).The causes of poisoning are combined medication,different diseases,etc.Conclusion:There are many influence factors of SDC which are different among individual patients.Different etiology, pathophysiology,factors, and combined drugs could affect serum digoxin concentration, so we should design the individual plan to take medicine,to guide reasonably uses the medicine.Pharmacist should play a greater role in the application of digoxin.
