平顶山市2004～2011年甲型病毒性肝炎流行特征分析

摘　　要：目的了解平顶山市甲型病毒性肝炎发病流行规律,为有效防控甲肝提供科学依据。方法采用描述流行病学方法分析,对甲肝三间分布、疫苗效果等进行比较。结果 1996～2011年平顶山市甲肝发病率波动在3.43/10万～17.40/10万之间,年均发病率为7.25/10万,2004～2011年共报告甲肝3249例,年均发病率为8.20/10万,2010年发病率最低3.55/10万,2007年最高17.40/10万。发病高峰在每年7～9月份,农村高于城市,两者发病率差异有统计学意义(χ2=281.46,P﹤0.001)。病例以10岁以下儿童为主,共1647例,占病例总数的50.69%,男性多于女性。职业以学生发病最多,为1186例,占发病总数的36.50%;其次为儿童和农民,分别占36.23%和21.42%。结论防治甲肝应采取以预防接种、疫情监测和健康教育为主的综合性防控措施。OBJECTIVE The study was conducted to analyze the epidemiological characteristics of viral hepatitis A in Pingdingshan and provide basis for prevention and control of viral hepatitis A. METHODS Descriptive epidemiological methods were adopted to analyze the immunological effect and the distribution of population, geography and time in viral hepatitis A cases. RESALTS From 1996 to 2011, the incidence of viral hepatitis A ranged from 3.43 / 100 000 to17.40 / 100 000 with the average annual incidence of 7.25 / 100 000. From 2004 to 2011, there were 3 249 cases, and the average annual incidence was 8.20 / 100 000 with the highest incidence of 17.40 / 100 000 in 2007 and the lowest one of 3.55 / 100 000 in 2010. The peak arranged from July to September. There was a statistical difference in the incidence between the urban and rural area （χ 2 = 281.46, P﹤0.001）, and the incidence was higher in rural area. Most cases were younger than 10 years of age , with a total of 1647 cases, accounting for 50.69%, and the incidence of males was higher compared with female. Students contributed the most to the caseload, with a total of 1186 cases, accounting for 36.50%, followed by children and farmers, respectively, accounting for 36.23% and 21.42%. CONCLUSIONS It is important to enhance inoculation, surveillance and health education for prevention and control of viral hepatitis A.
