去氨加压素治疗前列腺增生患者夜尿增多的临床研究

摘　　要：目的:观察小剂量去氨加压素治疗良性前列腺增生(benign proststic hyperplasia,BPH)患者夜尿增多的疗效及安全性。方法:采用前瞻性实验研究方法,将2012年1-10月我院收诊的前列腺增生合并有夜尿增多的患者,随机分为安慰剂对照组和去氨加压素治疗组,观察比较两组患者用药前后在夜尿次数、夜尿量及第一段睡眠持续时间方面的疗效。结果:共纳入55例患者,其中去氨加压素治疗组28例,安慰剂对照组27例,随访3周显示,去氨加压素治疗组在减少夜尿次数及夜尿量,延长第一段睡眠持续时间方面同安慰剂对照组差异具有统计学差异(P<0.01),且治疗过程中无严重不良反应发生。结论:对于BPH/LUTS患者的夜尿增多症状,给予小剂量去氨加压素治疗安全有效。Objective：To determine the efficacy and safety of low dose oral desmopressin in the treatment of nocturia in elderly patients with benign prostatic hyperplasia/ lower urinary tract symptoms（BPH/LUTS）.Methods：Fifty-five BPH/LUTS patients with nocturia treated in our hospital from Jan.to Oct.2012 were randomized into two groups.27 patients in the control group received placebo and 28 patients in treatment group received 0.1 mg desmopressin orally at bedtime,for 3 weeks＇ observation.Voiding diaries were assessed before and after the treatment.Results：In the desmopressin group,most patients reported a good response with both reduced nocturnal voids（3.6 vs.1.9,P0.01） and urine volume（689.6 mL vs.473.2 mL,P0.01）.The mean duration of the first sleep period was improved by 84 min（110.6 vs.194.2,P0.01）.Compared with the control group,the desmopressin group showed significantly reduced nocturia frequency（41% vs.9%;P0.01）,decreased nocturia urine output（31% vs.7%;P0.01）,prolonged first sleep period（76% vs.9%;P0.01）.In addition,there were no adverse actions during the treatment.Conclusion：Short-term low dose oral desmopressin is an effective and well tolerated treatment for nocturnal polyuria in the lower urinary tract symptoms of patients with benign prostatic hyperplasia.
