T-SPOT-TB法评价结核性脑膜炎疗效的应用研究

摘　　要：目的通过观察结核性脑膜炎患者正规抗结核治疗前和治疗1、3个月后外周血ELISPOT斑点数变化,从而间接监测患者病情发展变化,用以指导临床用药。方法分别收集结核性脑膜炎患者(30例)正规抗结核治疗前的外周血和治疗1、3个月后的外周血,分离提取单个核细胞,经冻存和复苏后,行T细胞酶联免疫斑点(T-Spot-TB)检测外周血中IFN-γ分泌细胞,统计其斑点数。结果正规抗结核治疗1、3个月后的外周血T-Spot-TB试验斑点数与治疗前比较明显减少,差异有统计学意义(P<0.05)。结论依据治疗前后患者外周血ELISPOT检测斑点数的变化,能够一定程度地反应结核性脑膜炎早中期临床治疗效果及疾病发展变化。Objective To observe the regular anti - tb treatment for tuberculous meningitis patients before and after treatment for 1 month and 3 month by measuring the change in the number of peripheral blood ELISPOT spots to get the purpose of indirectly moni- toring the changes in disease progression of the patients, so as to guide clinical medication. Methods Collected respectively peripheral blood of 30 cases with tuberculous meningitis, which were got in three different periods. That was before treatment, after treatment for one month and three months. Mononuclear cells were extracted and isolated from peripheral blood of the patients, through cryopreservation and recovery, IFN -gamma -secreting cells in the peripheral blood would be detected by T- cell ELISpot （T- Spot -TB）, then the number of spots was counted. Results The number of spots in peripheral blood from tuberculous meningitis patients who were after the regular anti - tuberculosis treatment in one month and three months were lower than that of the patients who were before treatment. There was sig- nificant difference （ P 〈 0.05 ）. Conclusion In the light of changes before and after the treatment of tuberculous meningitis in periph- eral blood of patients with ELISPOT detection of the number of spots, it would reflect the tuberculous meningitis clinical outcomes and disease development in changes of the early and middle stage to some extent.
