摘　　要：目的观察糖尿病肾病患者外周血中性粒细胞（PMNs）上细胞间黏附分子-1（ICAM-1）的表达情况及PMN-培养内皮细胞黏附率的变化，探讨外周血PMNs上ICAM-1作为糖尿病肾病机体炎症状态指标的临床意义。方法选取住院2型糖尿病患者94例，按尿白蛋白排泄率（UAER）分为3组（正常蛋白尿组63例、微量蛋白尿组20例和临床蛋白尿组11例），以及健康对照组30例，抽取外周血，分离中性粒细胞，免疫组化检测ICAM-1表达。并测定各组PMNs-培养内皮细胞黏附率变化。结果糖尿病患者各组中性粒细胞ICAM-1表达与PMNs-培养内皮细胞黏附率均屁著高于对照组，并且ICAM-1表达水平与尿白蛋白排泄率呈正相关。结论外周血PMNs上ICAM-1表达水平可作为糖尿病肾病机体炎症状态的评价指标，辅助指导临床治疗。Objective To investigate potential clinical application of ICAM-1 in polymorphonuclear neutrophils （PMNs） as a biomarker of inflammatory status in diabetic nephropathy by measuring ICAM-1 expression and PMN-cultured endo-thelial cell （EC） adhesion changes. Methods Ninety-four type 2 diabetic patients were divided into 3 groups （normal albuminuria, 63 patients; micro-albuminuria, 20 patients; clinical albuminuria, 11 patients） according to urinary albumin excretion rate （UAER） with 30 healthy volunteers as the control group. Peripheral blood was collected for evaluation of ICAM-1 expression using immunohistochemical method and PMN-cultured EC adhesion analysis. Results Compared to the control group, ICAM-1 expression and PMN-EC adhesion rate were elevated in the diabetic groups. A positive correlation between ICAM-1 expression and UAER was observed. Conclusion ICAM-1 expression in peripheral PMNs may be used as a biomarker for assessment of inflammatory status in patients with diabetic nephropathy.
