依达拉奉对高血压脑出血脑氧代谢及脑血管血液动力学参数的影响

摘　　要：目的分析依达拉奉对高血压脑出血脑氧代谢及脑血管血液动力学参数的影响。方法将2010年4月至2012年8月进行常规治疗的34例高血压脑出血患者设为对照组,以同期常规治疗加依达拉奉进行治疗的34例患者为观察组,然后将两组患者治疗前和治疗后不同时间的脑氧代谢指标[脑氧含量(SjvO2)、脑组织氧分压(PbtO2)、颈内静脉血氧含量(CjvO2)、脑氧摄取率(CEO2)及动脉-颈内静脉球部血氧差(Da-jvO2)]及脑血管血液动力学参数(Vmean、Qmean、R、DR及CP)进行比较。结果观察组治疗后不同时间的脑氧代谢指标中SjvO2、PbtO2及CjvO2均高于对照组;CEO2及Da-jvO2均低于对照组;Vmean及Qmean均高于对照组,而R、DR及CP均低于对照组,且观察组治疗后的3d和10d上述方面均优于治疗前(均P<0.05)。结论依达拉奉对高血压脑出血患者脑氧代谢及脑血管血液动力学参数的改善发挥着积极的作用。Objective To study and analyze the influence of edaravone for the cerebral oxygen metabolism and cerebral vascular hemodynamic parameters of patients with hypertensive cerebral hemorrhage. Methods 34 patients with hypertensive cerebral hemorrhage and treated with conventional therapy in the hospital from April 2010 to August 2012 were the control group,and 34 patients treated with conventional therapy and edaravone were the observation group,then the cerebral oxygen metabolism indexes（SjvO2 ,PbtO2 ,CjvO2 ,CEO2 and Da-jvO2 ） and cerebral vascular hemodynamic parameters（Vmean, Qmean,R,DR and CP） of two groups before and after the treatment at dif- ferent time were compared. Results The observation grouprs SjvO2 ,PbtO2 and CjvO2 of cerebral oxygen metabolism indexes after the treatment at different time were all higher than those of control group,while the CEO2 and Da-jvO2 were all lower than those of control group,and the Vmean and Qmean of cerebral vascular hemodynamic parameters were all higher than those of control group , but the R,DR and CP were lower than those of control group,and these indexes of observation group after the treatment at third and tenth day were all better than those items before the treatment（all P〈0.05）. Conclusion The edaravone plays an active role in improving the cerebral oxygen metabo lism and cerebral vascular hemodynamie parameters of patients with hypertensive cerebral hemorrhage.
