静脉应用维库溴铵预防TURBt闭孔神经反射的初步研究

摘　　要：目的:探讨膀胱侧壁肿瘤行经尿道肿瘤电切术(TURBt)中发生闭孔神经反射的预防措施。方法:65例行TURBt的膀胱侧壁肿瘤患者,硬脊膜外麻醉后,配合静脉麻醉加用肌松药物维库溴铵静脉注射,同时配合喉罩通气。结果:65例TURBt患者中,5例发生了术中患侧下肢的轻度抖动,其他患者未发生闭孔神经反射,1例发生膀胱穿孔,无1例发生麻醉并发症。结论:静脉应用维库溴铵配合喉罩通气预防膀胱肿瘤电切术中的闭孔神经反射是一种简单、安全、有效的方法。Objective： To investigate the method to prevent the obturator nerve reflex in the process of Transurethral Resection of Bladder Tumor（TURBt） of lateral wall tumor patients.Method： A total of 65 patients suffered from lateral wall bladder tumor underwent TURBt.After epidural anesthesia,performed intravenous anesthesia plus intravenous injection of muscle relaxant drugs vecuronium,and then,in conjunction with the laryngeal mask airway ventilation.Results： In 65 patients,mild adductor spasm occurred in 5 cases,other patients didn＇t have obturator nerve reflex,bladder perforation occurred in 1 case,and no cases had anesthesia complications.Conclusion： Intravenous vecuronium plus the laryngeal mask airway ventilation to prevent obturator nerve reflex in the TURBt of lateral wall bladder tumor is simple,safe,and effective.
