摘  要:目的 N-乙酰天门冬氨酸(NAA)定量研究对新生儿核黄疸的评价及诊断。方法 10例新生儿核黄疸和8例年龄匹配的健康足月儿纳入我们的研究,波谱采用采用德国西门子Sonata1.5T超导型核磁共振仪,多体素氢质子波谱脑检查,点分辨波谱(PRESS)序列,TR 1 500ms,TE 135ms,兴趣区为两侧基底节区。结果 10个新生儿核黄疸,7个在T1加权像可见苍白球对称性高信号,T2加权像可见苍白球呈等信号。新生儿核黄疸组基底节区NAA和NAA/Cr较对照组相比明显降低,两组差别有统计学意义(P﹤0.05)。新生儿核黄疸组基底节区CHO/NAA较对照组相比明显升高,两组差别有统计学意义(P﹤0.05)。新生儿核黄疸组基底节区NAA值分布范围,只有一例与对照组有交叉重叠,其余没有交叉重叠。结论氢质子波谱及常规MRI对于新生儿核黄疸的评价及诊断有一定的意义,NAA定量研究对新生儿核黄疸的评价也有一定的价值。Objective To evaluate the usefulness of NAA concentrations in diagnosis of neonatal kernicterus. Methods A total of 10 patients were enrolled in the neonatal kernicterus group and 8 healthy, age-matched neonates were included as controls. All patients and controls underwent 1H-MRS(echo time of 135 ms and a repetition time of 1 500 ms) and con- ventional magnetic resonance (MR) sequences. The spectroscopic regions of interest included the bilateral basal ganglia. Results Seven of the 10 neonatal kernicterus had Tl-weighted image high signal intensity in the globus pallidus and ap peared as normal intensity in the region for the T2-weighted images. NAA and NAA/Cr in the basal ganglia were found to be significantly lower for the neonatal kernicterus group compared to the control groups ( P 〈0.05). CHO/NAA in the basal ganglia was also found to be significantly lower for the neonatal kernicterus group compared to the control groups ( P 〈0.05). Only one datum was an overlap in the ranges of the NAA calculated for the globus pallidus for the neonatal ker-nicterus group and the control groups. Most have no overlap in the ranges of the NAA calculated for the basal ganglia for the neonatal kernicterus group and the control groups. Conclusion 1H-MRS may be useful for the diagnosis of the neonatal kernicterus.
