异位妊娠的治疗方法及疗效探讨

摘　　要：目的探讨三种方法治疗异位妊娠的疗效。方法将126例异位妊娠患者分为A(药物保守治疗)、B(腹腔镜手术)、C(常规剖腹手术)三组,对三组的手术时间、术中出血量、术后镇痛、术后住院天数及治愈率进行比较分析。结果 A、B、C组的平均住院天数分别为(24.5±15.3)、(4.5±2.0)、(7.5±4.0)d,B、C两组均较A组时间短,差异有统计学意义(P<0.05);B、C两组的治愈率分别为94.9%和100%,均高于A组的78.3%。结论根据患者及医院情况选择不同治疗方法,对手术顾虑大、无药物禁忌证、血β-人绒毛膜促性腺激素<2000 IU/L、妊娠孕囊直径≤2 cm、早期异位妊娠、有生育要求的患者宜选用药物保守治疗;对年龄小、有药物禁忌证、要求保留生育能力的患者应首选腹腔镜或开腹保守性治疗;对年龄大、无生育要求的患者以开腹或腹腔镜根治性手术为宜。Objective To compare the curative effect of three methods in treatment of ectopic pregnancy.Methods One hundred and twenty-six ectopic pregnancy patients were randomly divided into group A,group B and group C accordig to the drug conservative treatment,patients in group A were given conservative medication,patients in group B were treat by laparoscopic surgery,and patients in group C were treated by traditional laparotomy.and then compared the operation time,intraoperative blood loss,postoperative analgesia,postoperative hospitalization days and the cure rate among the three groups.Results The average hospitalization days were（24.5±15.3）,（4.5±2.0）,（7.5±4.0） in group A,B and C,respectively;and group B,C than tho in group A,the difference was statistically significant（P＆lt;0.05）.The cure rate were 94.9%and 100%in group A and B,respectively;which was higher than that in group A（78.3%）.Conclusion According to situation of patients and hospitals,patients should be giving different trerapeutic method.Patients are givig conservative medication if they have many anxiety of the operative,without contraindication,the level of β-hCG＆lt;2000 IU/L,the diameter of gestational sacs≤2 cm,early ectopic pregnancy,and women who want to preserve their reproductive capacity;Patients are taking laparotomy or by larparoscopy conservative treatment if they are younger,have drug contraindications and women who want to preserve their reproductive capacity.Patients are taking laparotomy or by larparoscopy radical surgery if they are elderly and have no fertility requirements.
