氟比洛芬酯联合罗哌卡因切口局部浸润在妇科腹腔镜术后镇痛中的应用

摘　　要：目的观察氟比洛芬酯联合罗哌卡因切口局部浸润在妇科腹腔镜术后镇痛的效果。方法选择60例择期行妇科腹腔镜术患者,将其随机分为氟比洛芬酯联合罗哌卡因局部浸润组(A组)30例和氟比洛芬酯联合生理盐水局部浸润组(B组)30例。两组均给予气管插管全身麻醉。记录两组患者术后4、8、12、24、48 h心率(HR)、平均动脉压(MAP)和视觉模拟评分(VAS),以及术后补救镇痛例数和不良反应。结果 B组术后4、8、12 h HR、MAP、VAS均明显高于A组(P<0.05);术后B组有7例患者给予曲马朵补救镇痛,出现恶心、呕吐6例,不良反应发生率20.0%(6/30),显著高于A组(P<0.05)。结论氟比洛芬酯联合罗哌卡因切口局部浸润在妇科腹腔镜术后镇痛中效果确切,不良反应少,值得临床应用。Objective To investigate the effects of incision local infiltration of flurbiprofen combined with ropivacaine used in postoperative analgesia after gynecological laparoscopy. Methods 60 patients undergoing selective gynecological la- paroscopy were selected and randomly divided into two groups. The flurbiprofen combined with ropivacaine incision local infil- tration group （A） and the normal saline incision local infiltration group （B）. Both groups were given the general anesthesia of tra- chea cannula. The heart rate （HR）, mean arterial pressure （MAP）, visual analogue scale （VAS） scores, case number of postopera- tive analgesic superaddition and the related adverse reactions in the two groups were observed and recorded at postoperative 4,8,12,24,48 h. Results HR, MAP and VAS scores at postoperative 4,8,12 h in the group B were obviously higher than those in the group A （P〈0.05） ;in the group B, 7 cases were added with tramado after operation and 6 cases appeared nausea and vomit- ing. The incidence rate 20.0% （6/30） of adverse reactions in the group B were significantly higher than that in the group A （P〈0.05）. Conclusion Incision local infiltration of flurbiprofen combined with ropivacaine has definite analgesic effects and little adverse reactions after gynecological laparoscopy, which is worthy of clinical application.
