宫颈液基细胞学中人乳头瘤病毒L1蛋白的表达及意义

摘　　要：目的探讨人乳头瘤病毒L1蛋白(HPVL1蛋白)在193例宫颈液基细胞学中的表达及意义。方法采用核酸水平和免疫水平双重检测193例宫颈液基细胞学中HPVL1蛋白的表达情况。结果 HPVL1蛋白在宫颈未见上皮内病变/恶性细胞(NILM)中阳性表达率为3.33%(4/120),在宫颈低度鳞状上皮内病变(LSIL)中阳性表达率为52.08%(25/48),差异有统计学意义(P<0.01);在宫颈高度鳞状上皮内病变(HSIL)中阳性表达率为16%(4/25),与在LSIL中表达比较,差异有统计学意义(P<0.01);HPVL1蛋白在≤45岁和>45岁者的阳性表达率分别为17.07%和15.71%,差异无统计学意义。结论 HPVL1蛋白表达异常在宫颈鳞状上皮内病变的发生、发展中具有重要的作用。HPVL1蛋白检测联合宫颈液基细胞学检测能更早地发现宫颈鳞状上皮内病变,HPVL1蛋白对宫颈鳞状上皮内病变有一定的预测作用。Objective To study the expression and significance of the HPVL1 protein in 193 cases of cervi-cal liquid-based cytology. Methods The expression of HPVL1 protein was studied in 193 cases of cervical liquid-based cytology from the immumohistochemical level and the nucleic acid level. Results The Positive expression rate of HPVL1 protein in cervical intraepithelial lesion or Malignancy （NILM） is 3.33% （4/ 120）, positive expression rate of HPVL1 protein in cervical low-grade squamous intraepithelial lesion （LSIL） is 52.08% （25/48） ; there is an significant statistical difference between NILM and LSIL （P〈 0.01）, positive expression rate of HPVL1 protein in cervical high-grade squamous intraepithelial lesion （HSIL） is 16% （4/25）, the significant statistical difference existed in LSIL and HS1L （P 〈0.01）, posi-tive rate of HPVL1 protein in not more than 45 years old testees and in 45 years old testees were 17.07 and 15.71% respectively, no significant difference existed. Conclusion Abnormal expression of HPVL1 protein may play an important role in initiation and development of cervical squamous intraepithelial lesion. Detection of HPVL1 protein and cervical liquid-based cytology can early find cervical squamous intraepithe-lial lesion, HPVL1 protein has a certain effect on the prediction cervical squamous intraepithelial lesion.
