儿童腹腔镜脾切除术35例分析

摘　　要：目的探讨腹腔镜下儿童脾切除术的手术方法及治疗经验。方法2007年4月至2011年8月我们对35例儿童实施腹腔镜下脾切除术。其中男18例，女17例，年龄4岁10个月至16岁3个月，平均年龄8岁7个月。结果35例中，2例因脾蒂血管出血而中转开腹手术，2例因脾脏巨大无法装入取物袋而于主操作孔处增加约5cm长辅助切口取出脾脏，其余患儿均顺利完成手术。手术时间70～180min，平均125min；术中失血15—200mL，平均25mL。35例患儿均恢复顺利，术后住院6～10d，平均7d。29例术后随访3～25个月无异常。结论腹腔镜下脾切除术安全可行，创伤小，患儿恢复快。Objetive To review our experience in Laparoscopic splenectomy in children. Methods - From Apri!2007 to June 2011,35 children （ 18 boys and 17 girls） aged from 4 years and 10 months to 16 years and 3 months （ mean 8 years and 7 months ） underwent laparoscopic splenectomy . Results The operation in the all cases had been performed successfully without any severe complications. The average time of the opera- tion was 125 minutes. The average bleeding volume during the procedure was 25 ml. Conclusion Laparo- scopic splenectomy is safety in children. And it is beneficial to be of lesser trauma during procedure and shorter time for recover.
