补阳还五汤联合西药治疗气虚血瘀型脑梗死随机平行对照研究

摘　　要：[目的]观察补阳还五汤联合西药治疗气虚血瘀型脑梗死疗效。[方法]使用随机平行对照方法,将60住院患者按按随机数字表法随机数字表法随机分为两组。对照组30例常规神经内科治疗,包括降低颅内压,改善脑微循环,营养脑细胞的神经细胞活化剂,抗血小板聚集,活血化瘀,控制血糖,调整血压,维持水电解质平衡及各种并发症等对症处理。治疗组30例补阳还五汤(黄芪60g,当归20g,赤芍15g,地龙15g,川芎20g,红花20g,桃仁15g,全蝎6g,地鳖虫6g,川牛膝20g),随证加减,诸药水煎,取汁约200mL,1剂/d,早晚分服,西药治疗同对照组。连续治疗7d为1疗程。观测神经功能缺损程度、肢体运动功能、日常生活活动能力(ADL)、不良反应。连续治疗2疗程,判定疗效。[结果]神经功能缺损评分、肢体运动功能FMA评分及BI指数两组均有改善(P<0.01),治疗组改善优于对照组(P<0.05,P<0.01)。[结论]补阳还五汤联合西药治疗气虚血瘀型脑梗死疗效满意,改善患者运动功能,减轻神经功能缺损程度,提高日常生活活动能力。[Objective]To bservation Buyang Huanwu Decoction and Blood cerebral infarction combined with western medicine.[Methods]Using random parallel control method,the 60 hospitalized patients were randomly divided into two groups by random number table random number table.The Department of Neurology,the control group,30 cases of conventional therapy,including reducing intracranial pressure,improving cerebral microcirculation,nutrition brain cells,nerve cells activator,anti-platelet aggregation,blood circulation,control blood sugar,blood pressure adjustment,maintaining water and electrolyte balance and various complications and other symptomatic treatment.Treatment group,30 patients BYHWD（Astragalus 60g,angelica 20g,red peony root 15g,earthworm 15g Chuanxiong 20g,safflower 20g,peach kernel 15g,the Scorpion 6g,land turtle worms 6g,Cyathula 20g）,with the card addition and subtraction of various drugs decoction,juice of about 200 mL,1 agent / d,morning and evening service,western medicine with the control group,continuous treatment 7d 1 course.Observing the degree of neurological deficit,limb motor function,activities of daily living ability（ADL）,adverse reactions.Continuous treatment course,to determine efficacy.[Results] Neurological deficit scores,limb motor function FMA score and BI index in both groups improved（P〈0.01）,improvement in the treatment group than the control group（P〈0.05,P〈0.01）.[Conclusion]Buyang Huanwu Decoction combined with Western medicine treatment of cerebral infarction satisfaction of Qi and Blood,improve motor function in patients to reduce the degree of neurological deficit,improve activities of daily living.
