睾丸不动精子行卵细胞浆内单精子注射的临床效果分析

摘　　要：目的探讨睾丸新鲜不活动精子行卵细胞浆内单精子注射(ICSI)的临床效果。方法对27例因梗阻性无精子症行睾丸穿刺取精手术获得不活动精子行ICSI的病例进行回顾分析,女方常规超促排卵,在取卵日男方行睾丸穿刺抽吸取精术,经体外处理孵育后未获得活动精子,选择尾部柔软、结构完整的精子行ICSI。以同期行手术取精获活动精子行IC-SI的160个病例为对照组。结果不动精子组与对照组两组病例在夫妇双方平均年龄、平均获卵数、平均获成熟卵子数、成熟卵子比例、平均移植胚胎数均无显著性差异(P>0.05),两组间的低受精发生率、可利用胚胎率、优胚率、平均移植优胚数、临床妊娠率、种植率与早期胚胎流产率无显著性差异(P>0.05),两组受精率(71.0%和80.9%)、卵裂率(93.5%和97.8%)与2PN受精率(80.0%和88.2%)均有非常显著性差异(P<0.001)。结论通过挑选尾部柔软的不动睾丸精子行ICSI,虽受精率受到较大影响,但临床妊娠情况令人满意,仍是睾丸无活动精子患者一种可行的治疗方法。Objective： To investigate the clinical effect of intracytoplasmic-sperm-injection（ICSI） with testicular immobile sperm.Methods： The outcomes of ICSI for 27 obstructive azoospermic patients who had their immobile sperm obtained by testicular sperm aspiration（TESA） were analyzed,in these cases,although conventional super promoting ovulation were operated on women and TESA on men but it still fail to achieve active sperm in vitro treatment after incubation,and sperm’s soft tail and integral structure were chosen.The control group consisted of 160 obstructive-azoospermic patients who had activities sperm obtained by ICSI in the same period.Results： There are no significant differences（P 0.05） on test and control group in couples with average age,average egg number,average number of mature eggs,mature eggs ratio,average number of embryos.And the same to the low fertilization rate,available embryo rate,superior embryo rate,average transplantation optimal embryo number,clinical pregnancy rate,planting rate and early embryo abortion rate.Two groups of fertilization rate（71.0% and 80.9%）,cleavage rate（93.5% 和 97.8%）,and 2PN fertilization rate（80.0% and 88.2%） were with significant differences（P 0.001）.Conclusions： Great influences maded through selecting the soft tail of fixed testicular sperm for ICSI.Clinical pregnancy is satisfactory,It’s a feasible treatment on patients with no activity sperms of testicular.
