低位切除结合高位松挂线分次紧线术治疗高位复杂性肛瘘的临床观察

摘　　要：目的:观察低位切除结合高位松挂线分次紧线术治疗高位复杂性肛瘘的临床疗效。方法:将80例高位复杂性肛瘘患者随机分为治疗组和对照组,每组40例。治疗组采用低位切除结合高位松挂线分次紧线术治疗,对照组采用低位切开高位挂线术治疗。观察两组患者的治愈时间、住院时间、治愈率、随访6个月的复发情况,采用Wexner计分法评价两组患者的肛门功能。结果:治疗组患者的住院时间较对照组明显缩短(P<0.01);两组患者的治愈时间、治愈率及复发情况比较,差异均无统计学意义(P>0.05)。术后1个月、6个月,治疗组患者的肛门功能明显优于对照组(P<0.05,P<0.01)。结论:低位切除结合高位松挂线分次紧线术治疗高位复杂性肛瘘可缩短住院时间,较好地保护患者的肛门功能。Objective： To evaluate the clinical effects of low resection and high loose seton with frapping thread gradually therapy in treating high complex anal fistula.Methods： Eighty patients with high complex anal fistula were randomly divided into two groups： the treatment group in which 40 cases were treated with low resection and high loose seton with frapping thread gradually therapy and the control group in which 40 cases were treated with low incision and high seton therapy.The cure time,hospitalization time,cure rate,and recurrence of 6-month follow-up were observed,and the function of anal sphincter was evaluated by Wexner method.Results： The hospitalization time of treatment group was shorter than that of control group（P＆lt;0.01）;there was no statistical difference on cure time,cure rate and recurrence between two groups（P＆gt;0.05）.One month and six months after operation,the anal function of patients in treatment group was better than control group（P＆lt;0.05,P＆lt;0.01）.Conclusion： Low resection and high loose seton with frapping thread gradually therapy has good effects on high complex anal fistula with shorter hospitalization time,and can protect the function of anal sphincter.
