补肾健脾方调控原发性肝癌患者细胞免疫功能及血管生成相关因子临床研究

摘　　要：目的观察补肾健脾方对脾虚兼肝肾阴虚型原发性肝癌患者细胞免疫功能及血管生成相关因子的影响,探讨补肾健脾方抑制原发性肝癌复发转移的机制。方法将符合纳入标准的60例原发性肝癌患者随机分为治疗组和对照组,每组30例;治疗组予补肾健脾方,对照组予复方斑蝥胶囊;两组均28天为1个疗程,共治疗2个疗程,观察细胞免疫功能(CD3+、CD4+、CD4+/CD8+、NK)以及血管生成相关因子(VEGF、MMP2、MMP9)的变化情况。结果与本组治疗前比较,治疗组CD3+、CD4+、CD4+/CD8+水平治疗后明显提高(P<0.05),对照组各指标差异均无统计学意义(P>0.05);组间治疗后比较,CD3+、CD4+差异有统计学意义(P<0.05)。与本组治疗前比较,治疗组治疗后VEGF、MMP9水平明显降低(P<0.05),对照组各指标差异无统计学意义(P>0.05);组间治疗后比较,各指标差异均无统计学意义(P>0.05)。结论补肾健脾方能够改善脾虚兼肝肾阴虚型原发性肝癌患者的细胞免疫功能,降低VEGF、MMP9的水平;补肾健脾方可能是通过改善原发性肝癌患者的免疫功能和抑制肿瘤血管新生发挥治疗作用。Objective To investigate the effects of ＂Bushen Jianpi Decoction＂ on cellular immune function and angiogenesis factor of patients with primary hepatic carcinoma（PHC） of spleen deficiency combined with liver-kidney yin deficiency pattern,and explore the mechanism.Methods Sixty cases were randomized into treatment group and control group.Thirty cases in treatment group were treated by ＂Bushen Jianpi Decoction＂,and 30 cases in control group were treated by ＂Fufang Banmao Capsule＂,with two courses（one course is 28 days）.The cellular immune function（CD3＋,CD4＋,CD4＋/CD8＋,NK） and angiogenesis factors（VEGF,MMP2,MMP9） were evaluated.Results After treatment,the levels of CD3＋,CD4＋ and CD4＋/CD8＋ were markedly increased in treatment group（P0.05）,and no significant difference was found in control group（P0.05）;after treatment,there were significant differences in CD3＋ and CD4＋ between treatment group and control group（P0.05）.The levels of VEGF and MMP9 were decreased in treatment group after treatment（P0.05）,and no significant difference was found in control group（P0.05）;no significant difference was found in the indicators between the two groups（P0.05）.Conclusion ＂Bushen Jianpi Decoction＂ can enhance the cellular immune function and decrease the levels of VEGF and MMP9;the therapeutic effect may be associated with the improvement of immunity and inhibition of angiogenesis.
