喜炎平联合丙种球蛋白在重症手足口病治疗中的应用

摘　　要：目的探讨喜炎平联合丙种球蛋白治疗重症手足口病的临床效果。方法选择四川省双流县第二人民医院2011年1月-2012年8月收治的重症手足口病患儿85例,随机分为对照组(40例)和观察组(45例)。对照组接受在常规治疗基础上,给予喜炎平注射液5mg/(kg·d),观察组增加给予丙种球蛋白1g/(kg·d),2d为一个疗程。观察并比较两组退热时间、疱疹消退时间、口腔炎愈合时间、神经受累时间和住院时间及临床效果。结果观察组退热时间、疱疹消退时间、口腔炎愈合时间、神经受累时间和住院时间分别为(3.5±1.1)d、(2.6±0.7)d、(2.5±0.7)d、(2.9±0.9)d和(3.3±1.5)d,均显著低于对照组(P<0.05),治疗后观察组显效25例,有效18例,无效仅2例,总有效率为95.6%,显著高于对照组(77.5%)(P<0.05)。结论喜炎平联合丙种球蛋白治疗重症手足口病效果显著,能显著缩短患儿病程、降温和临床症状改善时间。Objective To explore the clinical effect of Xiyanping combined with gamma globulin in the treatment of severe hand-foot-mouth disease. Methods 85 cases with hand-foot-mouth disease admitted to the hospital from January 2011 to August 2012 were randomly divided into two groups, 40 cases in the control group were given Xiyanping for 5 mg/（kg.d）, 45 cases in observation group were given intravenous immunoglobulin for 1 g/（kg.d）based on the control group and 2 days for a course. The cooling time, herpes subsided time, stomatitis healing time, nerve involvement, hospitalization time and effect of 2 groups were compared. Results The cooling time, regression time, healing time Of herpes stomatitis, nerve involvement time and hospital stay were （3.5±1.1）d, （2.6±0.7）d, （2.5±0.7）d, （2.9±0.9）d and （3.3±1.5）d, significantly lower than those in the control group （P〈0.05）, the observation group after treatment excellent in 25 cases, effective 18 cases, invalid 2 cases, the total effective rate was 95.6%, which was significantly higher than that in the control group （77.5%）（P〈0.05）. Conclusion The effect of Xiyanping combined with gamma globulin in the treatment of severe hand-foot-mouth disease significantly, can significantly shorten the duration in children, cooling and clinical symptom improvement time.
