60株医院感染洋葱伯克霍尔德菌的临床分布及耐药性分析

摘　　要：目的了解医院洋葱伯克霍尔德菌的临床分布及其耐药性变迁特征，指导临床更加合理使用抗菌药物，有效及时控制医院感染。方法对我院2007～2011年分离自临床住院患者的60株洋葱伯克霍尔德菌的临床分布及药敏结果进行回顾性分析。结果近五年来，我院洋葱伯克霍尔德菌分离株占临床细菌构成比呈逐年上升趋势，51．67％分离自ICU患者，且主要来源于痰液标本，主要引起免疫低下患者呼吸道感染。洋葱伯克霍尔德菌对多种抗生素存在天然耐药，但美洛配能、头孢哌酮，舒巴坦、左氧氟沙星、复方磺胺和头孢他啶对其仍有较好抗菌作用（R〈30％），可作为临床抗洋葱伯克霍尔德菌感染经验用药使用。结论高度重视临床耐药菌株监测。加强医院医疗器械和一线医护人员生物安全和手消毒，尤其ICU病房更应高度重视洋葱伯克霍尔德菌患者的隔离和治疗，避免医院感染的爆发流行。Objective To investigate the distribution characteristics and drug resistance of Burkholderia cepacia （BC） isolated in our hospital ,so as to provide the reference for clinical application of antibiotics to control hospital infection. Methods Drug resistance and clinical distribution of 60 strains of BC during from 2007 to 2011 were analyzed and studied retrospectively. Results The study showed the isolated rate of BC was increased year by year in our hospital , the strains were almost isolated from sputum, with coming from ICU more than half. The drug resistant rates to Meropenm, Cefoperazone/Sulbactam, Levofloxacin, Ceftazidime and Cotrimoxazole were also low （R 〈 30%）. Conclusion With drug-resistant strains monitoring , ICU should pay high attention to control the BC colonization and spread in order to avoid hospital infection outbreak at last.
