添加ω-3鱼油脂肪乳对腹部手术后肠黏膜通透性的影响

摘　　要：目的 观察鱼油脂肪乳对胃肠道手术后患者肠道黏膜通透性的影响.方法 60例接受胃肠道根治性手术的患者随机分为观察组和对照组,所有患者于术后第1天开始接受相同的营养支持治疗,观察组另加用10%鱼油脂肪乳100ml.分别于手术前1d和术后第1、6天检测肝功能指标、血浆二胺氧化酶（DAO）浓度和血浆D-乳酸活性.结果 两组患者术后肝功能指标的变化差异没有统计学意义.术后的DAO水平两组均明显高于术前,术后6d与术前相比,观察组的上升幅度（59%）明显小于对照组（104%）（P＜0.05）.术后6d观察组的血浆D-乳酸水平上升幅度（19倍）远小于对照组的26倍（P＜0.05）.结论 鱼油脂肪乳对手术患者的肝功能没有影响,能减轻腹部大手术肠黏膜的损害,保护肠黏膜屏障.Objective To investigate the effects of ω-3 fish oil fatty emulsion on the intestinal mucosa permeability after gastrointestinal surgery. Methods Sixty patients with gastrointestinal malignant neoplasm who underwent abdominal radical operation were randomly divided into study group and control group equally. All patients received the same nutritional support proposal at the 1 st day after operation. The ω-3 fish oil fatty emulsion（ 100 ml/d, 10% ） was appended to the study group meanwhile. The levels of ser-um liver function parameters ,diamine oxidase（DAO） and D-lactic acid were detected at the lst day before operation and the 1 st and 6th day after operation. Results The liver function parameters of the two groups had no differences. The concentrations of postoperative DAO in control group and study group were higher. The ranges of increase in study group at 6th day after operation were less than those of control group（ 59% vs. 104% ,P 〈 0.05 ）. The values of D-lactic acid at 6th day after operation in study group were lower than those in control group （ 19-fold vs. 26-fold,P 〈 0.05 ）. Conclusion There was no effect of fish oil fatty e- mulsion on liver function of patients after surgical operation. Fish oil fatty emulsion could postpone the in-testinal barrier impairment after major abdominal operation and protect the intestinal nmcosa barrier.
