摘  要:目的观察呼吸频率对兔心率变异(HRV)和血压的影响。方法建立兔控制呼吸模型,用呼吸机控制呼吸频率(40,50,60次/分)和潮气量(10 ml/kg),用生理记录仪记录呼吸、心电、血压波形。观察呼吸运动对RR间期(RRI)及血压的影响;呼吸频率对RRI及血压波动幅度的影响;呼吸频率对HRV、血压变异(BPV)、呼吸峰中心频率的影响;呼吸频率对HRV影响采用传统HRV和eHRV(enhanced HRV)方法分析并对两结果加以比较。结果①RRI、血压波动周期与呼吸周期呈同步变化;②与呼吸频率50次/分相比,40次/分使RRI及血压的波动幅度增大,60次/分使RRI及血压波动幅度减小;③HRV和BPV的呼吸峰中心频率相同,50次/分时高频峰在低频与高频交界处,与呼吸频率50次/分相比,40次/分使呼吸峰中心频率左移至低频段,60次/分使呼吸峰中心频率右移,完全在高频段;④传统HRV分析结果示:随呼吸频率增加LF、LFnorm逐渐减小,HF、HFnorm逐渐增大;eHRV分析结果示:与呼吸频率50次/分相比,40次/分使HFnorm(高频标化值)减小,LFnorm(低频标化值)和LF/HF增大;60次/分使HFnorm增大,但差异均无显著性。结论 RRI及血压的波动周期与呼吸周期相同,减慢呼吸频率使RRI及血压波动幅度增加,HRV、BPV呼吸峰中心频率左移,加快呼吸频率使RRI及血压波动幅度减小,HRV、BPV呼吸峰中心频率右移,但一定程度内的呼吸频率波动对eHRV分析结果无显著影响。Objective To observe the impact of respiratory frequency(RF) on heart rate variability(HRV) and blood pressure(BP). Methods Establish the rabbit model of controlled respiration, the ventilation rate and tidal volume were preset by breathing machine, SKY-A4 an electrophysiolograph was used to record respiratory, ECG and BP waveform. To observe the im- pact of inspiration and expiration on RR interval (RRI) and BP, of ventilation rate on RRI, BP, and the central frequency of respiratory peak, HRV frequency spectra. Two methods traditional and enhanced HRV were adopted to analyze HRV. Results ①RRI and BP cyclical change coincided with the respiratory cycle. ②ventilation rate could affect the amplitude of RRI and BP cyclical change. Compared with 50 bpm, 40 bpm could increase the amplitude and 60 bpm could decrease the amplitude of cyclical change. ③ the central frequency of respiratory peak would shift to right as ventilation rate increase. RF 40 bpm, it was totally located within the range of LF. 50 bpm, it was located near the junction of HF and LF. 60 bmp, it was totally located within the range of HF. ④raditional HRV analysis results: LF, LFnorm decreased, and HF, HFnorm were ventilation rate would not change. HRV frequency spectra significantly increased as respiratory frequency increasing. There was no statistical significance found in the HRV frequency spectra between different ventilation rate. Conclusion RRI and BP cyclical change coincides with the respiratory cycle, the amplitude of RRI and BP cyclical change would increase and the HRV respiratory peak would shift to left as ventilation rate decrease, the amplitude of RRI and BP cyclical change would decrease and the HRV respiratory peak would shift to right as ventilation rate increase. To some extend the change of ventilation rate would not change HRV frequency spectral significantly.
