经肛门微创手术治疗小儿先天性巨结肠症40例临床疗效分析

摘　　要：目的:探讨经肛门微创手术治疗小儿先天性巨结肠症的临床效果。方法:将我院收治的40例先天性巨结肠患儿,随机分为对照组和试验组,对照组采用经腹会阴联合手术治疗,试验组采用经肛门微创手术治疗。分析两组手术时间、术中出血量、恢复肠道排气时间、住院时间,并发症发生情况等。结果:组术中出血量、肠道恢复排气时间、住院时间均显著优于对照组,差异有统计学意义(P<0.05)。试验组手术时间与对照组差异无统计学意义(P>0.05)。试验组并发症发生率低于对照组,差异有统计学意义(P<0.05)。试验组术后排便控制功能显著优于对照组,差异有统计学意义(P<0.05)。结论:采用经肛门微创手术治疗小儿先天性巨结肠症,术后患者排便功能恢复好,手术情况佳,并发症少,疗效优于经腹会阴联合手术,值得临床推广应用。Objective： To study the clinical effect of the anal minimally invasive surgical treatment of pediatric congenital hirschsprung disease.Methods： 40 cases of congenital megacolon children,were randomly divided into control group and observation group.The control group in the abdominal perineal joint surgery treatment,and the observation group used minimally invasive surgical treatment of anal.Analysis operation time,intraoperative amount of bleeding,restore bowel evacuation time,length of hospital stay,complications,etc of two groups.Results： The amount of bleeding,intestinal recovery evacuation time,length of hospital stay of the observation group were significantly superior to control group,the difference was statistically significant（P0.05）.The observation group operation time and control group was statistically significant difference（P0.05）.The observation group complication rates significantly lower than those of the control group,the difference was statistically significant（P0.05）.The observation group postoperative bowel movement control function is significantly better than control group,the difference was statistically significant（P0.05）.Conclusions： The anal minimally invasive surgical treatment of pediatric congenital hirschsprung disease,curative effect is better than the abdominal perineal joint surgery,postoperative patients defecation function recovered well,operation condition good,complications,and less in,it is worth clinical application.
