摘　　要：[目的]运用失效模式与效应分析(FMEA)对护士医嘱处理流程进行改造,以提高医嘱执行的准确性,防范不良事件的发生。[方法]根据FMEA方法,成立流程改进工作小组,列出医嘱处理的流程及子流程,找出流程下可能出现的失效模式进行分析、评估,计算危机值,提出流程改造措施,观察改造后的成效。[结果]对医嘱处理流程的失效模式实施FMEA管理后危机值均较实施前显著降低。[结论]FMEA作为一种风险管理工具,能前瞻性地发现医嘱处理流程中存在的漏洞,提高护士执行医嘱的准确性。Objective:Using failure modes and effects analysis(FMEA) for transformation of nurses'medical advice processing flow,so as to improve the accuracy of medical advice executed and prevent the occurrence of adverse events.Methods:According to FMEA method,process improvement working group was set up,the flow process and sub-processes of medical advice were listed,and then to find the possible failure modes for analysis and evaluation,and calculating crisis value,then the process transformation was put forward,and then to observe the effectiveness after transformation.Results:Crisis value decreased significantly after implementation of FMEA management for failure modes of medical advice processing compared that before implementation of FMEA management.Conclusion:FMEA as a risk management tool can help nurses find out loopholes in medical advice processing prospectively and improve the accuracy of nurses executing medical advice.
