摘 要:目的探讨基质金属蛋白酶-2(MMP-2)和基质金属蛋白酶-9(MMP-9)在人腹主动脉瘤(AAA)和破裂腹主动脉瘤(RAAA)组织中的表达及其作用。方法应用苏木精-伊红染色及免疫组织化学方法,检测30例AAA组织(包括6例横径<5.0cm的小AAA,12例横径5.0~7.0cm的中AAA,6例横径>7.0cm的大AAA及6例RAAA)和7例正常腹主动脉组织(对照组)平滑肌细胞数目及炎细胞浸润情况;应用免疫组织化学方法检测各组MMP-2和MMP-9的表达,并采用图像分析技术进行定量分析。结果大AAA和RAAA组织的平滑肌细胞数量少于对照组、小AAA和中AAA(F=97.68,P<0.01)。大AAA和RAAA瘤壁淋巴细胞浸润与对照组相比增多(χ2=6.017、8.813,P<0.05),大AAA组和RAAA组瘤壁的巨噬细胞浸润高于对照组、小AAA组和中AAA组(F=41.05,q=4.014~16.179,P<0.01)。MMP-2在AAA组织中膜平滑肌细胞中表达,MMP-9在AAA组织平滑肌细胞和巨噬细胞中表达,AAA各组和RAAA组瘤壁MMP-2表达均明显高于对照组(F=11.27,q=3.522~8.241,P<0.01);小AAA组和中AAA组瘤壁MMP-2的表达高于大AAA组和RAAA组(q=3.394~5.226,P<0.01)。中AAA组、大AAA组和RAAA组MMP-9表达均明显高于对照组和小AAA组(F=7.06,q=4.197~6.114,P<0.05)。结论 MMP-2与AAA的形成及早期扩张密切相关,MMP-9与AAA的持续扩张及破裂密切相关。Objective To explore the expressions of matrix metalloproteinase-2 (MMP-2) and matrix metalloproteinase- 9 (MMP-9) and their actions in human abdominal aortic aneurysm (AAA) and ruptured AAA (RAAA). Methods HE staining and immunohistochemistry were applied to detect the numbers of smooth muscle cells (SMC) and inflammation cells in the tissue specimens of 30 AAA of various diameters (six small AAA: 〈5.0 era; 12 moderate AAA: 5.0--7.0 cm, six large AAA: 〉7.0 cm and six RAAA) and seven normal abdominal aorta (control group). The expressions of MMP2 and MMP-9 in each group were measured by immunohistochemistry, and image technique was employed for quantitative analysis. Results The numbers of SMC in large AAA and RAAA were less than that in the control group as well as small and moderate AAA (F= 97.68, P〈0.01). The wall of large AAA and RAAA contained more lymphocytes than the control group (x2 =6.017,8.813;P〈0.05). And the numbers of macrophages in large AAA and RAAA were more than that in the control, small and moderate AAA groups (F= 41.05, q = 4.014--16.179,P^0.01). The expressions of MMP-2 in the media layer of AAA in SMC, that of MMP 9 in SMC and macropha ges, that of MMP 2 in the wall of AAA in each group and RAAA were all higher than that in the control group (F=11.27,q= 3.522--8.241,P〈0.01). The expression of MMP 2 in small and moderate AAA was higher than in large AAA and RAAA (q= 3.394--5.226,P〈0.01), and that in moderate, iarge AAA and RAAA was higher than that in the small AAA and the control group (F=7.06,q=4.197--6.114,P〈0.05). Conclusion MMP-2 is closely associated with the formation and early expansion of AAA, and MMP 9 is closely related to lasting expansion and rupture of this condition.
