摘 要:目的构建大鼠不同分期的压疮缺血再灌注动物模型,并分析其缺血再灌注损伤相关指标的含量变化。方法将64只健康雄性SD大鼠随机分为正常对照组、控制对照组、Ⅰ期压疮组、Ⅱ期压疮组、Ⅲ期压疮组、Ⅳ期压疮组、可疑深部组织损伤组、难以分期压疮组,每组8只。采用臀肌下埋置铁片外加磁铁加压的方法构建大鼠压疮缺血再灌注模型,检测并比较各组超氧化物歧化酶(SOD)活性及丙二醛(MDA)、C反应蛋白(CRP)、局部渗出液中肿瘤坏死因子α(TNF-α)含量。结果大鼠各期压疮缺血再灌注动物模型均构建成功。与正常对照组比较,各期压疮组SOD活性明显下降,MDA、CRP含量明显增加,差异有显著性(F=55.54~156.90,q=5.026~38.744,P<0.05);各期压疮组之间比较,差异亦有显著性(q=4.086~33.642,P<0.05)。局部渗出液中TNF-α含量Ⅲ期压疮组低于Ⅳ期压疮组,差异有显著性(t=5.82,P<0.05)。结论构建6期压疮动物模型并明确各期压疮的生理变化,可为压疮相关实验研究和临床治疗提供理论依据。Objective To create models of different stage pressure ulcer ischemic-reperfusion (PUIR) in rats and analyze its related indicators. Methods Sixty-four Sprague Dawley rats were equally randomized to eight groups as follows., normal group, control group, stage-Ⅰ , stage-Ⅱ-, stage-Ⅲ- , stage Ⅳ-PU group, suspected-deep-tissue-injury group and hard-to-staging- PU group. A rat model of PUIR was created by implanting an iron plate in gluteus and pressed with magnetic iron from outside. The content of superoxide dicmutase (SOD), malondialdehyde (MDA), C-reactive protein (CRP) and tumor necrosis factor-α (TNF-α) in the local exudate in each group were detected. Results Each-stage rat PUIR models were successfully created. Com- pared with the normal group, the activity of SOD in each group of different stage PUIR was declined, while the content of MDA and CRP elevated, the differences being significant (F=55.54-156.90,q=5.026-38.744,P〈0.05); as compared between each PUIR group, the differences of that showed significance as well (q = 4.086-33.642, P 〈0.05). The content of TNF-α in local exudate in stage-Ⅲ-PU group was lower than that in the stage-Ⅳ-PU group (t = 5.82, P d0.05 ). Conclusion Constructing six- stage PU animal models and identifying physiological changes in different stages of PU can provide theoretical basis for PU-related experimental research and clinical therapy.
