摘  要:目的探讨门控心肌灌注断层显像(gated myocardial perfusion imaging,G-MPI)对冠状动脉慢血流(coronary slow flow,CSF)患者的临床价值。方法选择行冠状动脉造影(coronary arteriography,CAG)诊断为CSF患者58例为CSF组,同期行CAG示冠状动脉血流正常者61例为对照组。采用校正的TIMI血流帧数法(correct TIMIframe count,CTFC)评价冠状动脉血流速度。对2组患者分别行G-MPI检测,常规处理成断层图像,再应用定量门控分析软件(Quantitative gated SPECT,QGS)进行左心室功能定量分析。采用将左心室分为7个节段、20个亚段的5分制(0~4分)半定量方法 ,评估心肌受损的程度。结果 CSF组58例患者中56例阳性,共发现156个亚段异常,其中室壁运动异常患者32例;对照组61例中7例阳性,共发现18个亚段异常,室壁运动均未见异常。结论 G-MPI能准确探测CSF患者的心肌损害程度,对CSF患者的疗效判定及远期随访有重要的临床价值。Objective To investigate the clinical value of ^99Tc^m-MIBI gated myocardial perfusion imaging (G-MPI)in detecting coronary slow flow (CSF). Methods Fifty-eight patients with CSF were enrolled as CSF group, and sixty-one patients with normal coronary flow were enrolled as control group. Coronary flow patterns of the cases were assessed by corrected thrombolysis in myocardial infarction (TIMI) frame count (CTFC). All the patients received G-MPI examination, the data were treated into tomographic image ,quantitative gated SPECT(QGS) software was used for processing and interpretation. Myocardium of left ventricle was divided into 7 segments and 20 subsegments. A five-point scoring system was used to evaluate the myocardial damage. Results group, one huanderd and fifty-six subsegments of injured shown ventricular wall motion abnormality in group, 18 subsegments of injured myocardium motion abnormality in the group. Conclusion Fifty-six patients showed positive results in CSF myocardium were detected and 32 patients were this group; 7 patients showed positive results in control were detected and no patients were shown ventricular wall G-MPI can detect myocardial damage in CSF as a direct and non-invasive method, suggesting a greater clinical value in efficacy of observation,long term follow-up in patients with CSF.
