腹腔镜联合手术的围手术期处理

摘　　要：目的总结泌尿外科应用腹腔镜联合手术的体会以及围手术期处理经验。方法回顾性分析2004年8月至2011年6月，泌尿外科或联合其他科室应用经腹腹腔镜联合手术治疗76例腹部两处以上病变（泌尿外科两处疾病、泌尿外科加普外科或妇科疾病）患者的临床资料，并注重讨论手术体会和围手术期的处理。结果在遵循外科手术的基本原则的前提下，经术中相关医师密切配合、围手术期恰当处理，本组76例联合手术全部成功，无一例中转开腹，术后疼痛轻微、恢复顺利。所有患者出院后随访2月～2年，未发现腹腔感染、肠道粘连等相关并发症。结论应用经腹腹腔镜联合手术同期治疗腹部两处以上病变，通过合适的围手术期处理，可顺利完成手术及术后恢复。Objectives To summarize perioperative management of combined laparoscope surgery in urolo- gy. Methods 76 cases with abdomen diseases（ urological diseases or urological disease together with general sur- gery disease or gynaecology disease）underwent combined laparoscope surgery from August 2004 to June 2011 were analysed respectively. This article analysis of treated cases of laparoscopic surgery retrospectively, and focus on the discuss of perioperative handling experience. Results By appropriate perioperative management, all operations were completed successful of 76 cases, no one conversion to open, after operative have slight pain and recovery smoothly. All patients followed up 2 month -2year, did＇t found complications. Conclusions By appropriate perioperative management ,we can complete combined laparoscope surgery to treat different diseases in abdomen at the same time, to complete the surgery and postoperative recovery smoothly.
