小切口肾囊肿去顶术与后腹腔镜手术效果的对比分析

摘　　要：目的比较经腰小切口肾囊肿去顶术与后腹腔镜肾囊肿去顶术治疗单纯性肾囊肿的临床疗效及应用价值。方法选择2008年1月至2011年3月由北京市海淀医院泌尿外科收治的具有手术指征的单纯性肾囊肿患者92例,采用完全随机设计分为小切口组(43例)和后腹腔镜组(49例),比较两组患者的手术时间、术中出血量、术后住院天数及住院费用。结果小切口组与后腹腔镜组在手术时间、术中出血量方面比较差异无统计学意义(P>0.05),小切口组住院费用显著少于后腹腔镜组,术后住院天数多于后腹腔镜组,差异有统计学意义(P<0.01)。结论后腹腔镜肾囊肿去顶术是治疗单纯肾囊肿安全有效的手术方式,在不具备腹腔镜手术条件的情况下,可采用小切口肾囊肿去顶术。Objective To investigate the clinical significance and application value of retroperitoneal laparoscopic renal cyst unroofing and small incision renal cyst unroofing on simple renal cyst.Methods From Jan.2008 to Mar.2011,92 patients of simple renal cyst at Beijing Haidian Hospital were randomly divided into two groups,small incision group（43 cases） and retroperitoneal laparoscope group（49 cases）.The operative time,blood loss,hospital day and cost of hospitalization of the two groups were compared and analyzed.Results The two groups had no significant difference in operative time and blood loss（P〈0.05）,while small incision group had lower cost but longer postoperative hospital stay（P〈0.01）.Conclusion Retroperitoneal laparoscopic renal cyst unroofing is an effective and safe option for renal cyst,and small incision operating can be adopted when laparoscopic condition is not equipped.
