摘 要:目的探讨幽门螺杆菌(Hp)体外诱导耐药率情况,以便为临床用药提供依据。方法选取2株标准菌株和在陕西省第四人民医院保存的5株菌株共7株进行阿莫西林、甲硝唑、克拉霉素、四环素以及呋喃唑酮体外诱导耐药试验,随机选取2011年12月至2013年1月期间保存的135株菌株,运用琼脂稀释法测定最低抑菌浓度(MIC),对上述5种药物进行耐药率检测。结果体外诱导耐药试验中甲硝唑和四环素均诱导出5株菌株,但甲硝唑诱导倍数最多,阿莫西林和呋喃唑酮诱导的倍数均较低;在耐药率方面,甲硝唑的耐药性最高,而阿莫西林和呋喃唑酮最低,克拉霉素和四环素居中,其他四组与甲硝唑的耐药性比较,差异具有统计学意义(P<0.05)。结论 Hp对甲硝唑耐药最高,呋喃唑酮和阿莫西林敏感性最好,克拉霉素居中,除四环素外的其他四种抗生素均有助于预测Hp感染后抗生素耐药率变化的趋势。Objective To study the helicobacter pylori(Hp) in vitro induced resistance,so as to provide references for clinical medication.Method Seven strains including two standard strains and five strains preserved by the Fourth People's Hospital of Shaanxi Province were selected for amoxicillin,metronidazole,clarithromycin,etracycline and furazolidone in vitro induced resistance test.135 saved strains during December 2011-January 2013 were randomly selected,AGAR dilution method was used for determination of the minimum inhibition concentration(MIC),and then resistance rate test to the 5 above mentioned drugs was done.Results In vitro induced resistance test of metronidazole and tetracycline were both 5 strains,but metronidazole inducing factor was the biggest,amoxicillin and furazolidone inducing factors were low;as for the resistance rate,metronidazole resistance was the highest,and tamoschering and furazolidone were the lowest,clarithromycin and tetracycline were in the middle,the difference in resistance between metronidazole and the other four groups was statistically significant(P〈0.05).Conclusion Hp resistance to metronidazole is the highest,and its sensitivity to furazolidone and amoxicillin is the best,clarithromycin in the middle.Except tetracycline the other four antibiotics are all helpful to predict the Hp infected antibiotic resistance changing trend.
