摘　　要：观察α-硫辛酸(ALA)对2型糖尿病(T2DM)患者尿肿瘤坏死因子-α(TNF-α)排泄的影响,探讨其肾脏保护作用。36例T2DM患者(DM组)在原糖尿病治疗方案的基础上联合ALA,观察6个月。同期选择我院体检中心30名健康体检人员作为对照组(NC组),比较两组之间及ALA治疗前后空腹血浆葡萄糖(FPG)、糖化血红蛋白(HbA1c)、尿白蛋白(ALB)及尿TNF-α排泄的变化。结果显示:①基础状态时,DM组FPG、HbA1c水平及尿白蛋白肌酐比(UACR)、尿肿瘤坏死因子-α肌酐比(UTCR)均高于NC组(P<0.01);②与治疗前比较,DM组治疗后FPG和HbA1c无明显变化,UACR及UTCR较基线值明显下降(P<0.05);③UTCR与UACR水平呈显著正相关(r=0.706,P<0.01)。To observe the effect of alpha lipoic acid （ALA） treatment on urinary tumor necrosis factor-α （TNF-α） in type 2 diabetic patients and explore its possible reno-protective mechanisms. 36 of type 2 diabetic patients com- bined with ALA on the basis of anti-diabetes treatment were assigned as diabetes mellitus group （DM group）. At the same period,30 healthy subjects were selected as the normal control group（ NC group）. And FPG, HbA1 e, uri- nary albumin to creatinine ratio（UACR） and urine TNF-α to creatinine ratio （UTCR） were determined respectively between two groups, and at the basal and 6th month of the treatment. Compared to group NC, FPG, HbA1 c, UTCR and UACR increased significantly in group DM at the baseline（P 〈 0. 01 ）. Compared with pre-treatment, UACR and UTCR were all obviously decreased （ P 〈 0. 05 ） , DM group was positively associated with UACR（r =0 but FPG and HbAlc had no obvious changes. The UTCR in 706 ,P 〈0. 01 ） in diabetics.
