摘　　要：目的:探讨男性因素不孕妇女种植窗期血小板活化因子及其受体的表达与子宫内膜超声类型的关系。方法:选择男性因素不孕妇女自然周期尿LH峰日C型子宫内膜者10例为C型内膜组和A型子宫内膜者20例为A型内膜组。在种植窗期,对两组对象,采用ELISA法检测血清PAF的含量。流式细胞术检测外周血单核细胞上PAF-R的表达。免疫组织化学法检测PAF-R在子宫内膜及内膜血管中的表达。结果:男性因素不孕妇女C型子宫内膜组种植窗期血清PAF含量(13.97±1.70)ng/mL、PAF-R在外周血单核细胞上(63.49±5.36)%和在子宫内膜(0.30±0.03)、内膜血管(0.31±0.03)中的表达与A型子宫内膜组各项指标[(18.95±3.10)ng/mL、(77.56±6.58)%、(0.40±0.04)、(0.40±0.03)]分别比较,差异有统计学意义(均P<0.01)。两组妇女外周血单核细胞上PAF-R的表达与子宫内膜组织中PAF-R的表达均呈正相关。结论:男性因素不孕妇女子宫内膜超声类型与种植窗期PAF及PAF-R的表达有关。外周血单核细胞上PAF-R的表达水平可间接反映子宫内膜组织中PAF-R的表达情况。Objective To investigate the expressions of platelet-activating factor and its receptor at implantation window stage and their relationship with ultrasonographic pattern of the endometrium in infertile women for male factors.Methods 30 infertile women for male factors were divided into 2 groups according to their endometrium pattern on the day with peak value of urine LH（＋） in the natural cycle.10 women who exhibited a homogeneous pattern of the endometrium was included in pattern C group and 20 women who exhibited a triple-line pattern of the endometrium was included in pattern A group.During the period of implantation,DDDDDEEeExeEELISA assay was applied to analyze the level of PAF ;Expression of PAF-R on peripheral blood mononuclear cells（PBMCs） was examined by flow cytometry ;Expressions of PAF-R in endometrium and endometrial blood vessels were examined by immunohistochemistry.Results The serum concentration of PAF was（13.97 ± 1.70）ng /mL,the expression of PAF-R was（63.49 ± 5.36）% on PBMCs,0.30 ± 0.03 in endometrium,0.31 ± 0.03 in endometrial blood vessels in the pattern C group,which were significantly lower than those in the pattern A group [（18.95 ± 3.10）ng /mL,（77.56 ± 6.58）%,0.40 ± 0.04,0.40 ± 0.03]（all P〈0.01）.The expression of PAF-R on PBMCs was positively correlated with that in endometrium in both groups.Conclusion The ultrasonographic pattern of the endometrium in infertile women for male factors at implantation window stage was correlated with the concentration and expression of PAF and PAF-R.The expression of PAF-R on PBMCs can indirectly reflect the expression of PAF-R in the endometrium.
