丹参酮ⅡA对兔心肌梗死再灌注后无复流的干预

摘　　要：目的:观察丹参酮ⅡA对兔心肌梗死再灌注后无复流的影响,并探讨其作用机制。方法:新西兰大白兔40只,随机分成生理盐水灌注对照组(Ⅰ)、丹参酮再灌注治疗组(Ⅱ)、冠脉结扎前丹参酮治疗组(Ⅲ)、假手术组(Ⅳ),每组10只。制备急性心肌梗死再灌注模型。再灌注后观察左心室心肌梗死程度、无复流程度及ST段抬高指数。结果:①与Ⅰ组相比,Ⅱ组、Ⅲ组的梗死面积及无复流面积均较小(P<0.05),而Ⅱ组及Ⅲ组之间无明显差异(P>0.05)。②Ⅰ组、Ⅱ组及Ⅲ组组结扎前ST段抬高指数无明显差异(P>0.05),再灌注后Ⅱ组、Ⅲ组ST段抬高指数均小于Ⅰ组的(P<0.05),而Ⅱ组及Ⅲ组之间无明显差异(P>0.0)。结论:丹参酮ⅡA磺酸钠注射液能减小心肌梗死面积、减轻无复流的发生,减轻心肌损伤。Objective：To observe the effect and mechanism of tanshinonⅡA on the no-reflow phenomenon after reperfusion of acute myocardial infarction（AMI） in rabbits.Methods：A total of 40 New Zealand white rabbits were randomly divided into 4 groups：saline group（Ⅰ）,the group treated with tanshinonⅡA before reperfusion（Ⅱ）,the group treated with tanshinonⅡA before ligating coronary artery,and sham operation group（Ⅳ）.The model of myocardial ischemia reperfusion injury（MIRI）was produced.The no-reflow area（NRA） and the myocardial infarcted area（MIA） were determined by thioflavin S and TTC staining,respectively.And also,the ST-segment changes of electrocardiogram by the time of 120 minutes after reperfusion（expressed as∑STI） were observed.Results： ①GroupⅡand group Ⅲ resulted in a significantly smaller size in NRA and MIA in comparison with groupⅠ,while there was no significant difference in the effects between groupⅡand group Ⅲ.②There was no significant difference in ∑STI among groupⅠ,groupⅡ and group Ⅲ before ligating coronary artery.After reperfusion,groupⅡand group Ⅲ resulted in a significantly lower index in ∑STI in comparison with groupⅠ,but there was no significant difference between groupⅡand group Ⅲ.Conclusion： TanshinonⅡA could alleviate myocardial injury and improve no-reflow phenomenon in rabbits with experimentalacute coronary artery occlusion-reperfusion.
