创伤性骨折术后切口感染的影响因素及预防对策

摘　　要：目的探讨创伤性骨折术后切口感染的危险因素及预防对策,为创伤性骨折术后患者术后感染的预防提供依据。方法对医院2007年1月-2012年12月收治的1528例创伤性骨折患者作为研究对象,对导致患者切口感染的危险因素进行分析,并提出相应的预防对策。结果 1528例患者中发生感染54例,感染率为3.5%;发生切口感染的患者中不合理使用抗菌药物21例,其中术中未能及时追加抗菌药物9例,用药时间不合理7例以及抗菌药物选择不当5例;切口感染与手术切口类型、手术时间、损伤部位、损伤类型、年龄、是否合并糖尿病以及有无其他部位感染有明显的相关性(P<0.05)。结论手术切口的类型、手术时间、损伤部位、损伤类型、年龄、是否合并糖尿病以及有无其他部位是导致创伤性骨折患者切口感染的危险因素;保持引流管通畅、密切观察术后切口情况并做好营养支持等可作为预防创伤性骨折患者术后切口感染的预防对策。OBJECTIVE To investigate the risk factors and prevention countermeasures of the incision infections after traumatic fracture surgery so as to provide basis for the prevention of incision infections.METHODS A total of 1528 cases of traumatic fracture patients admitted to the hospital from Jan 2007 to Dec 2012 were selected as the study objects,then the risk factors leading to the incision infections were analyzed,and the corresponding countermeasures were put forward.RESULTS Of totally 1528 cases of patients,the incision infections occurred in 54cases with the infection rate of 3.5%,including 21 patients with the irrational use of antimicrobial agents,9 cases failed to additional antibiotics,7cases of irrational medication time,and 5 cases of unreasonable use of antibiotics.The incision infections were significantly related to the types of incision,operation duration,the sites of injury,types of injury,age,diabetes,and other sites of infections（P0.05）.CONCLUSION The types of incision,operation duration,the sites of injury,types of injury,age,diabetes,and other sites of infections are the risk factors leading to the incision infections in the traumatic fracture patients.To maintain the drainage tube unobstructed,closely observe the postoperative incision,and make nutritional support well done can be used as the prevention countermeasures for the postoperative incision infections in the traumatic fracture patients.
