摘 要:目的监测并分析某医院鲍氏不动杆菌(ABA)的流行状况及耐药性,为临床预防和治疗ABA感染提供依据。方法对某医院2009-2011年所分离ABA菌株的流行病学及耐药性进行回顾性分析。结果 3年共分离出ABA 895株,2009年检出病原菌3125株,ABA 144株,占4.61%;2010年检出病原菌2922株,其中ABA 440株,占15.06%;2011年检出病原菌2522株,其中ABA 311株,占12.33%;ICU 2009-2011年分离ABA 29、251、189株,分别占全院ABA的20.14%、57.05%、60.77%;体外药敏试验显示,ABA对各种抗菌药物的耐药率在逐年增加,其中头孢他啶、头孢吡肟、庆大霉素、亚胺培南、左氧氟沙星、哌拉西林/他唑巴坦耐药率均明显上升,3年总耐药率分别为50.81%、81.32%、87.01%。结论该院从2010年开始可能处于鲍氏不动杆菌的流行状态,其中重灾区为ICU,同样从2010年起该院鲍氏不动杆菌的耐药率也骤然升高,必须采取严格的消毒隔离措施阻断流行。OBJECTIVE To monitor and analyze the prevalence and drug resistance of Acinetobacter baumannii(A.baumannii)so as to provide basis for the clinical prevention and treatment of the A.baumannii infections.METHODS Retrospective analysis was performed to evaluate the epidemiological characteristics and drug resistance of A.baumannii in one hospital from 2009 to 2011.RESULTS A total of 895 strains of A.baumannii were isolated in three years,totally 3125 strains of pathogens were isolated in 2009,among which there were 144(4.61%) strains of A.baumannii;totally 2922 strains of pathogens were isolated in 2010,among which there were 440(15.06%)strains of A.baumannii;totally 2522 strains of pathogens were isolated in 2011,among which there were 311(12.33%)strains of A.baumannii.There were 29(20.14%)strains of A.baumannii isolated from ICU in 2009,251(57.05%)strains isolated in 2010,and 189(60.77%)strains isolated in 2011.The in vitro drug susceptibility testing showed that the drug resistance rate of the A.baumannii increased year by year,the drug resistance rates to ceftazidime,cefepime,gentamicin,imipenem,levofloxacin and piperacillin/tazobactam increased significantly,and the total drug resistance rates of the three years were 50.81%,81.32%,and 87.01%,respectively.CONCLUSION The hospital may be in the epidemic status of A.baumannii since 2010,most prevalent in ICU;the drug resistance rate of the A.baumannii is significantly increased from 2010.It is necessary to take rigid disinfection and isolation measures to block the prevalence.
