摘  要:目的总结联合应用常规超滤、零平衡超滤与改良超滤在婴幼儿体外循环手术中的经验、方法及临床效果。方法本组60例,男34例,女26例,年龄4~24个月,体重5~14kg,体外循环术中根据灌注压、红细胞压积(Hct)行常规超滤+零平衡超滤,停机后行改良超滤。结果 60例患儿均顺利停机,无死亡,未出现超滤并发症。转中Hct22%~28%,平均(25.2±2.2)%;停机改良超滤后Hct上升至33%~38%,平均(34.8±1.2)%。结论在婴幼儿体外循环中将常规超滤、零平衡超滤与改良超滤联合应用,零平衡超滤能排除炎性因子,改良超滤能在ECC结束后排出多余的水分,提高红细胞压积,改善心肺功能。Objective To summarize the experience,the method and the clinical effects of combined conventional ultrafiltration(CUF),zero balanced ultrafiltration(ZBUF) and modified ultrafiltration(MUF) in infant during extracorporeal circulation(ECC) operation.Methods Sixty infants including 34 males and 26 females,with 4~24 months of age and 5~ 14kg of weight were included.CUF and ZBUF were done during ECC by a variety of hematocrit(HCT) and mean arterial pressure(MAP) measurement,MUF was done after ECC.Results All patients recovered without severe complications.Hct of patients was(25.2±2)% during ECC and 34.8±1.2% after MUF.Conclusion Combined CUF、ZBUF and MUF is popular in infants during extracorporeal circulation operation.ZBUF can suppress inflammatory cytokines,while MUF can improve the respiratory and heart function through decreasing the body water and increasing hematocrit.
