高通量血液透析联合血液灌流对维持性血液透析患者皮肤瘙痒症的影响

摘　　要：目的研究高通量血液透析(HFD)联合血液灌流(HP)对维持性血透患者皮肤瘙痒症的影响。方法 40例血透患者随机分为对照组(HFD)和观察组(HFD+HP),在试验前及试验1、3个月后分别对两组患者瘙痒症状进行评分,同时检测透前Cr、BUN、P、Ca、iPTH、ALB、CRP等指标并进行比较。结果观察组较对照组瘙痒症状在1个月、3个月时均明显改善[(5±1)分vs.(6±1)分;(3±1)分vs.(5±1)分],差异有统计学意义(P<0.05);血清BUN、Cr、Ca差异无统计学意义(P>0.05);但CRP、ALB、iPTH、P在2种血液净化模式下有明显不同,差异有统计学意义(P<0.05)。结论 HFD+HP能减轻患者的皮肤瘙痒,降低炎症指标,改善营养不良,进而可能会提高血液透析者的生存质量。Objective To study the effects of high flux hemodialysis（HFD） with hemope rfushion（HP） on skin pruitus in hemodialysis patients.Methods Forty patients were randomly divided into the control group（HFD） and the study group（HFD＋HP）.The severity of skin pruitus,the serum Cr,BUN P,Ca,iPTH,ALB,CRP before dialysis between the two groups before the study and 1,3months after being enrolled into this study.Results The skin pruitus was improved significantly after treated by HFD ＋HP 1 and 3 months（P 0.05）.There were no differences in serum Cr,BUN,Caconcentrations between the two groups（P 0.05）.But the differences of the serum CRP,ALB,iPTH,P were statistically significant（P 0.05）.Conclusion The study shows that HFD＋HP can improve skin pruritus,depress inflammation,improve life quality in hemodialysis patients.
