摘 要:目的探讨前列腺增生患者(BPH)膀胱逼尿肌功能与收缩蛋白表达变化的关系。方法选择51例BPH患者,按尿动力检查结果分为逼尿肌功能正常组30例,逼尿肌不稳定组21例。选择同期15例非尿路梗阻患者为对照组,收集各组患者膀胱逼尿肌标本。应用免疫组化方法检测逼尿肌肌球蛋白、肌动蛋白的表达情况,比较各组之间的差异。结果肌球蛋白在对照组、BPH逼尿肌稳定组和逼尿肌不稳定组表达的阳性率分别为46.67%、66.67%和95.24%,组间差异有统计学意义(P<0.05)。肌动蛋白在对照组、BPH逼尿肌稳定组和逼尿肌不稳定组表达的阳性率分别为53.33%、73.33%和85.71%,组间差异有统计学意义(P<0.05)。结论膀胱出口梗阻后逼尿肌中肌球蛋白、肌动蛋白表达的增加可能是其收缩功能改变的分子基础。Objective To research the relationship of detrusor dysfunction and contractile protein in patients with Benign Prostate Hyperplasia(BPH).Methods Fifty-one cases of BPH patients based on urodynamic examination results were divided into the normal detrusor function group,in which 30 cases were enrolled,and the detrusor instability group,in which 21 cases were enrolled,15 cases withuot urinary tract obstruction patients were used as the control group.Bladder detrusor specimens were collected.Immunohistochemical method was adapted to detect the expression of the myosin and actin in detrusor,at the same time,the difference between groups was compared.Results The positive expression rates of myosin in the control group,BPH detrusor stability group and detrusor instability group were 46.67%,66.67% and 95.24% respectively,the differences between groups were statistically significant(P 〈0.05).The positive expression rates of actin in the control group,BPH detrusor stability group and detrusor instability group were 53.33%,73.33% and 85.71% respectively,differences between the groups were statistically significant(P 〈0.05).Conclution The increased expression of myosin,actin may be the molecular basis of the detrusor function change after bladder outlet obstruction.
