2HRZE/4HR方案联合胸腺肽、左氧氟沙星治疗初治涂阳肺结核的临床疗效分析

摘　　要：目的比较研究2HRZE/4HR方案联合胸腺肽、左氧氟沙星治疗初治涂阳肺结核的临床疗效,为临床用药提供参考。方法将104例初治涂阳肺结核患者随机分成两组,每组52例,观察组采用2HRZE/4HR化疗方案并联合胸腺肽、左氧氟沙星,对照组予2HRZE/4HR抗结核,观察两组治疗后痰菌阴转、病灶吸收情况、不良反应发生率及治疗转归。结果观察组2、4、6月末痰菌转阴率分别为90.38%(47/52)、94.23%(49/52)和96.15%(50/52),显著高于对照组,差异有统计学意义(P<0.05);且观察组在2、6月末的病灶吸收情况也明显好于对照组,差异有统计学意义(P<0.05)。观察组初治涂阳肺结核患者的总治愈好转率为96.15%,完全吸收88.46%,明显高于对照组,差异具有统计学意义(P<0.05)。结论 2HRZE/4HR方案联合胸腺肽、左氧氟沙星治疗初治涂阳肺结核可以明显提高痰菌阴转率及病灶吸收率,治疗转归方面比传统2HRZE/4HR方案好,临床上值得推广。Objective To evaluate clinical efficacy of 2HRZE/4HR anti-TB combined with thymosin and levofloxacin in patients with smear positive tuberculosis. Methods 104 cases of smear positive TB patients retreating in our hospital were randomly selected and allocated to observation group and control group. The observation group was treated with 2HRZE/4HR anti-TB combined with thymosin and levofloxacin, while the control group was treated with 2HRZE/4HR. The sputum negative conversion rate, focusing absorption rate, adverse reaction of the treatment, and treatment effect in the two groups were analyzed. Results In observation group, sputum negative conversion rates in 2, 4 and 6 month were 47 （90.38%）, 49 （94.23%） and 50（96.15%）, respectively, and were significantly higher than those of control group （P〈0.05）. The observation group′s absorption rate was also significantly higher than that of the control group （P〈0.05）. The total cure rate and absorption rate in the observation group was 96.15% and 88.46% , significantly higher than control group （P〈0.05）. Conclusion The sputum negative conversion rate, focusing absorption rate and curative effect in patients with smear positive tuberculosis were improved by 2HRZE/4HR anti-TB combined with thymosin and levofloxacin. It is worthy to be used in clinical.
