基层医院新生儿听力筛查网络系统的应用分析

摘　　要：目的探讨基层医院推广应用新生儿听力筛查网络系统的实用性和先进性。方法将广东省新生儿听力筛查网络管理系统应用于广东省中山市中医院和中山市十一所镇级医院出生的新生儿听力筛查管理,比较使用系统前(出生新生儿29 657例)、后(出生新生儿31 169例)新生儿听力筛查的初筛率、复筛率、转诊率、失访率、诊断率、干预率。结果使用系统前、后新生儿听力初筛率分别为92.8%、95.3%,复筛率分别为70.3%、88.9%,初筛失访率分别为29.7%、11.1%,转诊率分别为81.1%、93.1%,第二次诊断率分别为82.6%、91.3%,第一次转诊失访率分别为18.9%、6.9%,第二次转诊失访率分别为17.4%、8.7%,干预率分别为85.9%、94.1%。使用系统后的初筛率、复筛率、转诊率、诊断率和干预率较使用系统前明显增高(均为P<0.05),失访率较使用系统前明显降低(P<0.05)。结论新生儿听力筛查网络系统实用于基层医院新生儿听力筛查的管理,有利于提高其新生儿听力初筛率、诊断率和干预率,降低失访率。Objective To explore the utility and advancement of the neonate hearing screening net system（NHSNS）that applied in basic hospitals.Methods The neonate hearing screening system of the Guangdong Province was implemented in 12hospitals in Zhongshan City.A total of 29 657neonates born in the 12hospitals before the NHSNS application,who were included as group A,while,31 169neonates born after the NHSNS application were included as group B.The first screening rate,re-screening rate,referral rate,lose follow-up rate,diagnosis rate,and intervention rate were comparel between the two groups.Results The screening rate was 92.8% and 95.3%in the two groups respectively.Along with 70.3%and 88.9% of re-screening rate,29.7% and 11.1% of the first lose follow-up rate,81.1%and 93.1% of referral rate,82.6%and 91.3% of the second diagnosing rate,18.9%and 6.9% of the first referral lose follow-up rate,17.4%and 8.7%of the second referral lose follow-up rate,85.9%and 94.1% of the intervention rate.There were significant differences of the first screening rate,rescreening rate,referral rate,diagnosis rate,intervention rate between the two groups（P0.05）and significant differences of the lose follow-up rate（P0.05）.Conclusion The wide application of NHSNS in the basic hospitals flow sheet of the hearing screening center can promote the neonate hearing screening level.
