乳腺增生超声造影特征表现及诊断

摘　　要：目的探讨实时超声造影检查对乳腺增生病灶的诊断及鉴别价值。方法选用SonoVue超声造影剂,对经手术及病理证实的54例(58个肿块)乳腺增生肿块患者进行彩色多普勒超声造影,对肿块的增强形态、强度及造影时间-强度曲线特征进行分型,并将其超声造影的特征与病理组织学特征进行对照分析。结果乳腺增生组58个肿块造影强度分别为高增强14个,等增强16个,低增强18个,极少至无10个,内部可呈均匀及不均匀增强,造影剂进入肿块的大小及范围与二维超声图像基本一致,小部分超出,囊性增生特征常表现为中间无回声的边界环状增强或半环状增强;偶见声像图类似纤维腺瘤并钙化;该组时间-强度曲线常呈"慢上快下及快上快下"型;超声造影诊断良性肿瘤的符合率为85.1%(46/54)。结论乳腺增生肿块在超声造影下可呈现一定的特征,超声造影有助于提高乳腺病灶的鉴别诊断及诊断符合率。Objective To observe the value of real-time contrast enhanced ultrasonography in diagnosis of Hy-perplasia of Breast. Methods Totally 54 patients of breast lumps, including 58 breast lumps, were ohserved by color Doppler real time contrast entranced ultrasonography using SonoVue. The enhancement form, intensity and the time-intensity curve were classified respectively, and characteristics of the benign and malignant carcinoma were analyzed. Results The 58 lumps of Hyperplasia of Breast contrast-enhanced intensity, 14 cases were high enhancement, 16 were equal enhaneemen, 18 were low enhancemen and 10 were no enhancement, Inhomogeneous partial enhancement and entire enhancement appeared in these breast lumps, Sometimes, the uhrasonography of Hyperplasia of breast lumps with a wide range calcium deposition just like Breast Fibroadenoma. The time-intensity curve of the group dis played ＂slow washin and fast washout＂ and ＂fast washin and washout＂ The accuracy of contrast enhanced ultrasonog-raphy were 85.1%（46/54）. Conclusions The enhancement modes have different characters in Hyperplasia of Breast. Real time contrast-enhanced ultrasound helps to improve the identification diagnostic and accuracy of Hyperplasia of Breast.
