摘  要:目的探讨胎儿超声心动图诊断先天性冠状动脉瘘的方法和可能性。方法总结我院胎儿超声心动图诊断并经出生后超声证实的3例胎儿冠状动脉瘘的诊断方法和超声表现,分析二维超声心动图(2D)、彩色多普勒血流显像(CDFI)、连续多普勒(CW),以及后期运用E-flow血流显像技术诊断该病的特征。结果结合2D、CDFI、CW、E-flow多种显像方法能够在产前清晰显示冠状动脉瘘的起源、瘘管的走行以及瘘口的位置,并评估相应的血流动力学改变。结论胎儿超声心动图能够在产前诊断冠状动脉瘘,对出生后的早期诊断、早期治疗有重要意义。Objective To investigate the value of fetal echocardiography in diagnosis of fetal coronary artery fis-tulas. Methods 3 fetuses with coronary artery fistula diagnosed by echocardiography were included in the study, which were approved by follow-up. Echocardiography appearance and diagnostic methods,including the characteristics of the two-dimensional echocardiography(2DE), color Doppler flow imaging(CDFI), continuous-wave Doppler(CW) and E-flow imaging technique were concluded. Results It is legible to combine the different echocardiographic meth-ods to establish the origin, course and drainage of the fetal coronary artery fistula or to assess the shunt flow and the consequent hemodynamic changes. Conclusions Fetal echocardiography was able to diagnose the fetal coronary artery fistula in early embryonic phase, which was valuable to the early diagnosis and treatment for the disease.
