Evaluation of Cerebral Hemodynamic Differences Between Healthy Young Han People with Rapid Accession into Tibet and Tibetan Native Healthy Youth Using Ultrasound

摘  要:目的采用超声评价急速入藏的世居平原汉族健康青年和世居高原藏族健康青年脑血流动力学参数的差异。资料与方法选取急速入藏汉族健康青年66例,藏族健康青年29例,测量受试者的年龄、身高、肺活量体重指数、血红蛋白含量(HGB)、动脉血氧饱和度(SaO2)、外周动脉收缩压(SAP)、舒张压(DAP)、心率(HR),应用彩色多普勒超声技术记录受试者双侧颈内动脉、椎动脉和大脑中动脉收缩期峰值流速(PSV)、舒张末期血流速度(EDV)、平均流速(Vm)、阻力指数(RI),分析汉族青年和藏族青年间以及不同性别受试者间各参数的差异。结果汉族男、女青年肺活量体重指数(t男=1.706,t女=2.025,P<0.05)、HGB(t男=3.845,t女=11.918,P<0.05)、SaO2(t男=1.680,t女=3.349,P<0.05)均低于藏族男、女青年,SAP(t男=2.475,t女=4.172,P<0.05)、DAP(t男=4.414,t女=4.485,P<0.05)、HR(t男=1.915,t女=7.042,P<0.05)均高于藏族男、女青年;汉族青年颈内动脉、椎动脉RI均小于藏族青年(P<0.05);汉族男青年双侧Vm(t左=2.020,t右=1.827,P<0.05)、左侧PSV(t=5.500,P<0.05)、左侧血流量(VOL)(t=1.740,P<0.05)和右侧EDV(t=3.215,P<0.05)与藏族男青年比较差异有统计学意义;汉族女青年双侧EDV(t左=5.109,t右=3.534,P<0.05)、左侧Vm(t=4.157,P<0.05)和左侧VOL(t=3.103,P<0.05)均大于同侧藏族女青年。结论超声诊断技术可以无创、实时检测高原脑血流动力学的变化趋势,为高原相关性血流动力学研究提供参考依据。Purpose To evaluate the cerebral hemodynamic differences between native plain-living healthy young Han people with rapid accession into Tibet and native plateau- living Tibetan healthy youth using ultrasound. Materials and Methods 66 healthy young Han people with rapid accession into Tibet and 29 healthy young native Tibetans were enrolled, age, height, vital capacity body mass index, hemoglobin (HGB), arterial oxygen saturation (SaO2), peripheral artery systolic pressure (SAP), diastolic pressure (DAP) and heart rate (HR) were tested, peak systolic velocity (PSV), end diastolic velocity (EDV), mean velocity (Vm), resistance index (RI) of subjects' bilateral internal carotid artery, vertebral artery and middle cerebral artery were recorded with color Doppler ultrasound. Difference of each parameter between Han youth and Tibetan youth, and between subjects with different sexes were analyzed. Results FVC body mass index (tmale=1.706, t female=2.025, P〈0.05), HGB (t male=3.845, tfemale=11.918, P〈0.05), SaO2(t male=1.680, tfemale=3.349, P〈0.05) of both Han male and female youth were lower than those of their Tibetan cohorts, but their SAP (tmale=2.475, tfemale=4.172, P〈0.05), DAP (tmale=4.414, tfemale=4.485, P〈0.05) and HR (tmale=1.915, tfemale=7.042, P〈0.05) were higher than the Tibetan male and female youth; carotid artery and vertebral artery RI of the Han youth were less than those of the Tibetan youth (P〈0.05); there was significant difference of bilateral Vm (tleft=2.020, tright=1.827, P〈0.05), left PSV (t=5.500, P〈0.05), left blood flow (VOL) (t=-1.740, P〈0.05) and right EDV (t=3.215, P〈0.05) between young Han males and Tibetan young males; Bilateral EDV (tleft=5.109, tright=3.534, P〈0.05), left Vm (t=4.157, P〈0.05) and left VOL (t=3.103, P〈0.05) of young Han females were greater than the ipsilateral results of their Tibetan cohorts.
