摘 要:目的探讨包皮套环在自制通道单孔腹腔镜精索静脉高位结扎手术操作中的体会及红外热像在评估手术疗效中的价值。方法对我院2012年1月至2012年9月间48例左侧精索静脉曲张的患者随机分为两组,24例实施自制通道的单孔腹腔镜下精索静脉高位结扎手术,24例实施常规腹腔镜下精索静脉高位结扎术,通过红外热像、精液检查等评估手术效果。结果所有患者手术顺利,无并发症出现。单孔组和常规组分别有19例和21例患者获得随访,时间3~6个月。两组术后睾丸温度、精液质量改善明显(P<0.001),术后3月及6月均较术前明显改善,以术后6月改善最明显,睾丸体积无变化(P>0.05),不同组间测量指标无差异(P>0.05)。结论包皮套环自制通道在单孔腹腔镜手术中操作方便、可行,有推广价值,红外热像能够客观、有效评估手术效果及睾丸功能恢复情况。Objective To explore the effect of high ligation of spermatic vein under self-made single- hole laparoscope for varicocele, and the value of infrared thermography in evaluation of the temperature of the testicles. Methods From January 2012 to September 2012, there were forty-eight patients with left varico- cele enrolled in this study, all patients were randomly divided into two groups, 24 cases in each. One group un- derwent self-made single-hole laparoscopy and the other underwent traditional laparoseopy. Infrared thermogra- phy and spermatic fluid testing were used to evaluate the different surgery. Results All the operations were successful without complication. There was significant difference in testicular temperature and semen quality a- nalysis in each group( P 〈0.001 ) and no significant difference in testieular volume( P 〉 0.05 ). Measurement index between the two groups had no difference ( P 〉 0.05 ). Conclusions The self-made trocar with circum- cision device in the transperitoneal single-site surgeries is feasible. It has a wonderful cosmetic effect compared with the traditional procedure and can be recommended. The surgery and the function of testis could be objec- tively evaluated by infrared thermography.
