摘 要:目的观察核因子(NF)-κB和诱导型一氧比氮合酶(iNOS)在脓毒症大鼠心肌细胞的表达,同时探讨辛伐他汀对其表达的影响,以及他汀类药物的心肌保护性作用。方法雌性Wistar大鼠30只随机分为三组:正常组(N组),盲肠结扎穿孔术组(CLP组)和辛伐他汀组(S组),处理方法:①S组:喂养辛伐他汀20 mg/kg每天一次共2 w。②2 w后S组与CLP组大鼠行盲肠结扎穿孔术。③术后48 h取各组大鼠心肌组织制成石蜡切片,采用HE染色和免疫组织化学染色的方法检测心肌纤维iNOS和NF-κB的蛋白表达。结果 HE染色显示CLP组大鼠心肌纤维水肿,疏松,间质充血水肿,而S组心肌的组织病理学改变较CLP组轻。免疫组织化学染色显示CLP组大鼠心肌细胞iNOS与NF-κB蛋白表达于术后48 h较N组明显增加(P<0.05),S组大鼠心肌细胞iNOS与NF-κB蛋白表达与CLP组相比明显减弱(P<0.05),与N组相比无明显差异(P>0.05)。结论辛伐他汀可以减弱脓毒症大鼠心肌细胞iNOS和NF-κB的蛋白表达,改善脓毒症时的心脏功能。Objective To investigate the expressions of NF-κB and iNOS in cardiac myocytes of rats with sepsis and the effects of simvastatin on this process,and then discuss the heart protective effects of statins.Methods Thirty female Wistar rats were used in this study.The animal were randomly divided into 3 groups(n=10):normal(N);cecal ligation and puncture(CLP) and simvastatin(S) groups.Rats in S group were treated with simvastatin of 20 mg/kg once a day for two weeks before operating.Rats′ abdomen in S and CLP groups was opened,then the caecum was ligated and punctured.Samples of cardiac tissues of three groups at 48h after CLP were collected,and all the samples were made to paraffin sections for HE staining and immunohistochemistry(IH)staining,which were applied to examine the expression of iNOS and NF-κB.Results In the CLP group,myocardial fibers were edema and curmbly,myocardial fiber mesenchymal was congestive and edema.In the S group,the histopathology changes were not as serious as that of CLP group.The expressions of iNOS and NF-κB in CLP group were significantly higher than those of group N at 48 h after CLP(P0.05).In group S,the expressions of iNOS and NF-κB were weaker than those of CLP group(P0.05),and there was no difference between N and S group(P0.05).Conclusions Simvastatin could attenuate the expression of iNOS and NF-κB in cardiac myocytes of rats in sepsis,so it is thought that simvastatin may improve heart function in sepsis partially.
