HBsAg阳性母亲免疫球蛋白联合乙肝疫苗规律接种效果分析

摘　　要：目的探讨HBsAg阳性母亲免疫球蛋白联合乙肝疫苗规律接种效果。方法选择130例患者,根据是否规律接种分为两组,其中观察组孕妇在孕28周及预产期前1周注射乙肝免疫球蛋白200 IU,新生儿按照体重使用0.5ml/kg,145 IU/ml乙肝免疫球蛋白注射并在出生时、出生后第1、6个月分别注射乙肝疫苗5μg,对照组则因为各种原因,在总计母体2次注射,新生儿4次注射中存在1次以上的未注射,在新生儿12月龄和18月龄分别检测小儿乙肝表面抗原抗体情况,并统计两组抗体滴度。结果 12月和18月龄时观察组乙肝表面抗原阳性检出率低于对照组(P<0.05),乙肝表面抗体阳性检出率高于对照组(P<0.05),且12月龄和18月龄乙肝表面抗体均高于对照组(P<0.05)。结论在孕中期和孕晚期规律注射乙肝免疫球蛋白并在新生儿出生后实施乙肝疫苗和免疫球蛋白联合注射能较好的达到母婴阻断效果。Objective To explore the protective efficacy of hepatitis B immunoglobulin（HBIG） combined with hepatitis B vaccine on infants from the mothers being positive for hepatitis B surface antigen（HBsAg）.Methods One hundred and thirty infants were divided into two groups according to whether or not to be vaccinated regularly.Mothers of observation group were given two doses of 200 IU HBIG（at 28 weeks of pregnancy and 1 week before giving birth）,and their newborns received one dose of HBIG（at birth） and three doses of 5μg HB vaccine（at birth and in 1 and 6 months of age）.Mothers of control group were given two doses of 200 IU HBIG,while their newborns received an irregular vaccination.Serum HBsAg,antibody to HBsAg（anti-HBs） and antibody titer in all the infants were determined at 12 and 18 months of age.Results Among the infants at 12 and 18 months of age,the positive rates of HBsAg in observation group were both lower than those in control group（P〈0.05）,while the positive rates of anti-HBs in observation group were both higher than those in control group（P〈0.05）.The antibody titers of infants at 12 and 18 months of age in observation group were both higher than those of control group（P〈0.05）.Conclusions Through vaccination by three doses of HB vaccine and three doses of HBIG,majority of the infants achieved a protective level of anti-HBs at 12 and 18 months of age.
