不同时间注射艾赛那肽对2型糖尿病患者胃肠道反应的影响

摘　　要：目的观察不同时间使用艾塞那肽对患者胃肠道反应的影响。方法选择磺脲类药物治疗不达标的2型糖尿病患者30例,分别在餐前1h(试验组16例)及餐前即刻(对照组14例)加用艾塞那肽皮下注射,均2次/日,维持2个月,第1个月每次5μg,次月每次10μg,观察治疗前后两组相关代谢指标,对胃肠道反应按食欲、恶心、呕吐分度统计分析。结果与治疗前对比,两组糖化血红蛋白下降程度无统计学差异(P>0.05);胃肠道反应均在4周左右逐渐缓解,在第5周加量后部分患者再次出现食欲下降及恶心症状,但第6周均已消失;首先消失的症状是呕吐,而食欲改善最慢;与对照组比较,试验组胃肠道反应评价各项指标改善时间更早(P<0.05);两组均无严重低血糖事件发生。结论餐前1 h注射艾塞那肽可能对该药引发胃肠道反应的患者更适合。Objective To observe the effects of exenatide at different time on gastrointestinal reaction in type 2 diabetes millitus（ T2DM）. Methods Thirty cases of T2DM without reaching standard by sulfonylureas were selected and added exenatide at 1 h before meal（ experimental group,16 cases） and immediately before meal（ control group,14 cases）, twice daily for 2 months,5 μg each time at the first month and 10 μg each time at the second month.The related metabolic indexes before and after treatment were observed in the two groups.The gastointestinal reactions were statistically analyzed according to the degrees of appetite,nausea and vomiting.Results Compared with before treatment,the two groups had no statistical difference in the decrease degrees of hemoglobin A 1c（ P 0.05）; the gastrointestinal tract reactions were gradually alleviated in 4 weeks,after adding the exenatide dose in the fifth week,appetite drop and nausea symptom appeared again in some cases,but in the sixth week these symptoms disappeared;vomiting first disappeared and appetite was improved most slowly;compared with the control group,the various evaluation indicators of gastrointestinal reactions in the experimental group were improved earlier;no severe hypoglycemia events occurred in the two groups.Conclusion Exenatide by injection at 1 h before meal may be more suitable for the patients with gastrointestinal reactions induced by this drug.
